|
1. 頭頸部がん根治治療後(手術あるいは化学放射線治療を用いた)再発または初回時から遠隔転移を有する頸部癌症例(病理型は問わない)。 抗PD-1抗体(ニボルマブおよびペンブロリズマブ)が投与された症例とする。 (なかでも特に、外頸動脈分枝である顔面動脈、舌動脈、顎動脈などによる栄養血管が明確な症例) 2. 標準的外科的切除が不可能である患者 3.病期の分類「UICC TNM 悪性腫瘍の分類 第 7 版」による) 頭頸部癌:cTx Nx M0ないし1 4. Performance status(PS)が 0-2(ECOG 基準)である患者 5. 試験参加同意取得日の年齢が 20 歳以上である患者 6. 主要臓器(骨髄、心、肺、肝、腎等)に高度な障害がなく、臨床検査値(Day 1 前 4 週間以内) が以下の基準を満たすこと 好中球数:1,500 /μL 以上 へモグロビン値:10.0 g/dL 以上 血小板数:100,000 /μL 以上 血清総ビリルビン値:1.5 mg/dL 以下 7. AST(GOT)、ALT(GPT)値:施設基準値上限×2.5 以下 8. 腎機能低下者血清クレアチニン:1.2 mg/dL 以下 eGFR 40ml/min以上 9.動脈血酸素飽和度:95%以上(室温) 心電図:治療を要する異常所見を認めない 10. 効果判定の対象になる病変を有する患者 11. 生検による腫瘍組織検体が採取できる患者 12.本研究について同意説明文書を用いて説明を行い同意を取得した者 |
1. Head and neck cancer with recurrence or distant metastasis(any pathological type).Patients must have received anti-PD-1 antibodies (nivolumab and pembrolizumab). (especially in patients with well-defined nutritional arteries such as the external carotid artery branches of the facial, lingual, and maxillary arteries) 2. Patients for whom standard surgical resection is not feasible 3. Staging according to UICC TNM Classification of Malignant Tumors, 7th Edition Head and neck cancer: cTxNxM0 or M1 4.Patients with a performance status (PS) of 0-2 (ECOG criteria) 5.Patients who are at least 20 years of age on the date of consent to participate in the study 6.Patients must be free of severe impairment of major organs (bone marrow, heart, lungs, liver, kidney, etc.) and meet the following criteria for laboratory values (within 4 weeks prior to Day 1) 7.AST (GOT), ALT (GPT): not more than 2.5 times the upper limit of the facility standard 8.Serum creatinine of patients with impaired renal function: 1.2 mg/dl or less eGFR 40 ml/min or more 9. Arterial blood oxygen saturation: 95%or more (room temperature) EKG: no abnormal findings requiring treatment 10. Patients with lesions to be evaluated for efficacy 11. Patients from whom tumor tissue samples can be obtained by biopsy 12.Patients who have given an explanation of this study using the consent document and obtained consent. |

1. HIV 抗体陽性患者 2. HCV 抗体陽性患者 3. HBs 抗原が陽性又は HBV-DNA がリアルタイム PCR 法により「検出」と報告された患者 4. 自己免疫疾患(既往を含む)患者 5. 間質性肺疾患(疑いを含む)患者 6. 過去に抗体製剤の投与により重篤な過敏症の既往歴を有する患者 7. 複数の高血圧治療薬を服用しているにも関わらず、コントロールが困難な高血圧患者(収縮期血圧が 160 mmHg 以上又は拡張期血圧が 100 mmHg 以上) 8. コントロール困難な内分泌障害を有する患者 9. Day 1 前 4 週間以内に、生ワクチン又は弱毒化ワクチンの接種を受けた、又は受ける予定のある患者 10. 活動性の炎症性腸疾患又は下痢を伴うその他の重症な慢性胃腸疾患を有する患者 11. 1 型糖尿病の患者又はインスリンや経口血糖降下剤を継続使用しても、登録時 HbA1cがJDS(日本糖尿病学会の規準)値で6.5%以上または NGSP(国際標準)値で 6.9%以上の糖尿病を合併している患者 12. 不安定狭心症(Day 1 前 3 週間以内に発症または発作が増悪している狭心症)を合併,または Day 1 前 6 か月以内の心筋梗塞の既往を有する患者 13. 重複がんを有する患者。ただし、重複がんとは、同時性重複がん及び無病期間が 5 年以内の 異時性重複がんである。局所治療により治癒と判断される Carcinoma in situ(上皮内癌) もしくは粘膜内癌相当の病変は重複がんに含めないこととする 14. 副腎皮質ステロイド剤の持続全身投与、免疫抑制剤及び免疫強化剤の投与から Day 1 まで 4週間以上経過していない患者 15. Day 1 前 12 週間以内に腫瘍に対する免疫療法(腫瘍ワクチン等)が実施された患者 16. 妊娠中、授乳中(授乳を中止して再開しない場合を除く)の女性、又は男女を問わず同意取得時から治験薬投与終了後 6 か月間、コンドームなどの避妊方法による避妊に同意できない 患者(閉経後(最終月経から 1 年以上経過)又は外科的不妊術を施行した女性及び外科的避 妊術を施行した男性は除く) 17. 感染症、活動性炎症性疾患を合併している、又は発熱等の臨床症状があり感染症が疑われる患者 18. 精神障害又は認知症を有し、適切な同意取得に支障を来す患者 19. 全身性のステロイド剤投与による治療の継続が必要な患者 20. 造血幹細胞移植等の移植療法が実施された患者 21. 投与開始前 4 週間以内に他の治験薬を投与された患者 22.その他、研究責任医師または研究分担医師が本研究を実施するにあたり不適当と判断した者 |

HIV antibody-positive patients 2.HCV antibody-positive patients 3.Patients who are HBs antigen positive or HBV-DNA is reported as "detected" by real-time PCR 4.Patients with autoimmune diseases (including pre-existing autoimmune diseases) 5.Patients with interstitial lung disease (including suspected interstitial lung disease) 6.Patients with a history of serious hypersensitivity to antibody products in the past 7. Patients with hypertension that is difficult to control despite taking multiple antihypertensives (systolic blood pressure > 160 mmHg or diastolic blood pressure > 100 mmHg) 8. Patients with endocrine disorders that are difficult to control 9. Patients who have received or will receive a live vaccine or attenuated vaccine within 4 weeks prior to Day 1 10. Patients with active inflammatory bowel disease or other severe chronic gastrointestinal disease with diarrhea 11. Patients with type 1 diabetes mellitus or patients with diabetes mellitus with HbA1c of 6.5% or higher by JDS (Japanese Diabetes Society criteria) or 6.9% or higher by NGSP (international standard) at the time of registration despite continuous use of insulin or oral hypoglycemic agents 12. Patients with unstable angina (angina with onset or worsening within 3 weeks prior to Day 1) or a history of myocardial infarction within 6 months prior to Day 1 13.Patients with multiple cancers. Patients with multiple cancers are defined as synchronous multiple cancers and iatrogenic multiple cancers with a disease-free interval of 5 years or less. Carcinoma in situ (intraepithelial carcinoma) or intramucosal carcinoma that is considered curable by local treatment is not included in overlapping cancers. 14. Patients who have not received continuous systemic administration of corticosteroids, immunosuppressive or immunoenhancing drugs for more than 4 weeks prior to Day 1 15.Patients who have received immunotherapy (e.g., tumor vaccine) within 12 weeks prior to Day 1 16. Women who are pregnant or lactating (except when lactation has been discontinued and will not be resumed), or patients, regardless of gender, who are unable to consent to contraception using a condom or other contraceptive method from the time of consent until 6 months after completion of study drug administration (excluding postmenopausal women (1 year or more since the last menstruation) or women who have undergone surgical sterilization or men who have undergone surgical sterilization). Men who have undergone surgical sterilization are excluded. 17.) 17. Patients with infectious diseases, active inflammatory diseases, or suspected infectious diseases with clinical symptoms such as fever 18. Patients with mental disorders or dementia that would interfere with proper consent 19. Patients who require continued treatment with systemic steroids 20. ・研究自体(全体)の中止について 研究代表医師、CRBまたは規制当局が研究を中止するよう決定した場合とする。 ・個々の患者の中止基準等について 以下のように基準を設定する。 1. 登録後30日以内にプロトコール治療が開始できなかった場合 2. 研究対象者が同意を撤回した場合 3. 登録後、不適格が判明しプロトコール治療続行が研究対象者の不利益になると判断された場合 4. 研究対象者がプロトコール治療の中止を申し出た場合 5. 原疾患の増悪が認められた場合 6. 疾病等により、薬剤(ニボルマブ)が投与中止となった場合 7. 研究責任医師または研究分担医師が安全性を考慮し、プロトコール治療を継続できないと判断した場合 8. プロトコール治療期間中の何らかの事情による転院 9. 追跡不能:研究対象者が来院せず、連絡も取れなかった場合 この場合、連絡を取ろうと試みたことを原資料に記録する。 10.女性研究対象者の妊娠が判明した場合 |

対象患者に、局所麻酔下浅側頭動脈からのカテーテル挿入を行う。その上で、免疫チェックポイント阻害剤の動注を行う。具体的にはカテーテル挿入に関してはアンスロンP-Uカテーテルを用い、さらにステアリングマイクロカテーテルも用いることも考慮する。薬剤に関してはニボルマブ20mg(1バイアル2ml)を生食18mlと混ぜ、総量20mlとし、30分かけて動注する。 |
The subject patient undergoes catheter insertion through the superficial temporal artery under local anesthesia. Then, an immune checkpoint inhibitor is injected into the catheter. Specifically, the patient will be catheterized with an Anthrone P-U catheter, and a steering microcatheter will also be considered. Nivolumab 20 mg (2 ml per vial) will be mixed with 18 ml of saline to make a total volume of 20 ml, which will be infused over 30 minutes. |
