神経線維腫症II型に対するベバシズマブの有効性及び安全性を検討する 多施設共同二重盲検無作為化比較試験 | ||

神経線維腫症2型(NF2) は遺伝性・進行性の難病であり、両側の聴神経腫瘍を含む、中枢神経系の多発性腫瘍を来す。腫瘍の病理組織型は良性腫瘍であるが、多数の腫瘍が生涯に渡って発生することで機能障害が進行し、最終的に生命を奪う結果に繋がる。これまで有効な薬物治療は皆無であり、手術による摘出あるいは放射線治療のみが治療法であった。特に両側に生じる聴神経腫瘍は、増大に伴って脳幹圧迫など生命に関わる一方、聴力の低下・消失に至るため、患者にとって極めて大きな問題である。手術療法・放射線治療はいずれも聴力の温存が困難であり、こうした治療介入時点で聴力について多くの場合諦める必要がある。一方、近年ベバシズマブ(BV)治療のNF2の聴神経腫瘍に対する有効性が数多く指摘されるようになった。驚くべきことに、約半数に聴力が改善し、腫瘍の縮小効果も認められる。現在BVは悪性神経膠腫など数種類の癌腫に対して認可されているがNF2に保険適応がない。患者会から実用化の要望を受けて、我々は医師主導治験による認可取得を目指しており、これまでPMDAから対面助言を受けるなど準備を進めた。今回、多施設共同・無作為化・二重盲検・比較試験を行って、NF2に対するBVの有効性・安全性を評価する。本治験ではBV群およびプラセボ群の2群に割り付け24週時点の聴力改善効果を比較する(主要評価項目;最高語音明瞭度の改善割合)。24週以降48週までは両群ともBVの投与を行う。この初期治療有効例については聴力再増悪時にBVの再投与を行い、再治療の効果と安全性についても評価する。これまでの報告は後ろ向き試験が多数を占め、プラセボ対照無作為化試験が行われた事例はなく、かつ前向き臨床試験は最大のものでも14例の単アーム試験に留まっている。2群60例を組み入れる本試験は、世界初・最大規模の信頼性の高い前向き無作為化試験となる。BVがNF2症例における有効性と安全性が確認されれば、進行性の聴力悪化に苦しむNF2患者にとって画期的な治療法となり、NF2に対する治療の枠組み全体を刷新できる。薬物による聴力改善および腫瘍縮小は、これまで得ることのできなかった画期的効果であり、侵襲性の高い手術や定位放射線治療に比較して患者および家族の負担の軽減が期待される。 | ||

Background Neurofibromatosis type 2 (NF2) is a rare genetic hereditary disease characterized by multiple central nervous system (CNS) tumors, most frequently bilateral acoustic neuromas (vestibular schwannomas). Although the tumor possesses a benign pathological characteristic, behavior of NF2 is not benign at all. It arises in the CNS in large number through the life, affecting vital functions and affect the quality of life. NF2 are progressive, increasing gradually in size, making the life of patients fairly short. There have not been any chemotherapeutic agents available for clinical use so far, and surgery and/or radiotherapy are the only therapeutic options available now. Both treatment options, however, do not alleviate hearing function in NF2 patients with acoustic neuromas, but even exacerbate hearing loss. Consequently, most of NF2 patients suffered from hearing loss and become deaf due to bilateral acoustic neuromas. Introduction of new drugs is mandatory due to the urgent need to preserve their quality of life. Efficacy of bevacizumab for NF2 was reported for the first time in human in 2009, followed by many reports with striking results demonstrating that the drug improves hearing function significantly and decrease the size of acoustic neuromas in approximately half of the treated NF2 patients. Bevacizumab is an anti-vascular endothelial growth factor (VEGF) antibody and has been approved and insurance-covered for various cancers, such as non-small cell lung cancers, colorectal cancers, breast cancers, and malignant gliomas. NF2 patient groups have demanded strongly for bevacizumab as a treatment option for them and demonstrated their request to both Japanese government (MHLW) and us. We conducted a small clinical study by ourselves based on an advice made by MHLW, showing that bevacizumab had similar effect on Japanese NF2 patients as well. Bevacizumab would have a significant impact on the treatment of NF2 patients, changing even treatment paradigm, in which bevacizumab would be the first-line treatment, followed by conventional treatments (surgery and radiotherapy) only after bevacizumab fails. 2. Purpose and treatment protocols The purpose, or the goal of this clinical trial is to obtain an approval of prescribing bevacizumab for NF2 patients from the Japanese health authorities, aiming to preserve hearing function of those patients. The primary objective of this trial, in order to achieve the above goal, is to prove efficacy and safety of bevacizumab treatment for NF2 patients, specifically, to demonstrate that bevacizumab significantly improves hearing function of NF2 patients with acoustic neuromas and to assess the safety profile of the treatment as well. Design of the trial is a multicenter randomized double-blind control study (the total number=60). NF2 patients who have at least one acoustic neuroma that has not been treated by radiotherapy and, at the same time, hearing test of the same side of ear meets a criterion; maximum word recognition scores 80% or less, and pure tone averages less than 100dB. The primary endpoint is the ratio of patients with hearing improvement, whom are evaluated by word recognition score, at the time point of 24 weeks after starting treatment. In secondary endpoint, the following factors will be assessed: tumor volume, hearing functions evaluated by the pure-tone audiometry and the auditory steady-state response, Japanese NF2 severity score, and efficacy of re-challenge for responders in the primary treatment. | ||
