/ | CSR初版のデータカットオフ日(2019年3月17日)より前に登録が完了した。CSR初版の完成後、治験責任医師によるデータの再評価により、基準値の特性が一部変更された。1例の被験者では、直近のプラチナレジメンの最良治療効果がCRからPRに変更された。さらに、当該被験者は他の部位(腸間膜)に転移性病変を有していた。 被験者の年齢は44歳~79歳であった。年齢の中央値は62.0歳であり、卵巣癌の好発年齢である50歳超と一致した。平均体重は57.47 kgであり、海外第3相試験(NOVA試験)の対象集団の平均体重(69.69 kg)を下回った。 最後から2番目のプラチナ製剤投与後の進行までの期間は、12ヵ月超が最も多かった(14例、73.7%)。登録したすべての被験者が、最後(直近)のプラチナレジメン施行中にCR(8例、42.1%)又はPR(11例、57.9%)を達成していた。全19例が骨髄抑制の既往を有していた。ECOG performance status は、19例中17例(89.5%)が0、残り2例(10.5%)が1と報告された。登録した被験者の原発腫瘍部位は、卵巣(10例、52.6%)、腹膜(5例、26.3%)及び卵管(4例、21.1%)であった。ベースラインの平均血小板数(標準偏差)は214.1×10^9/L(58.06×10^9/L)であった。 CSR補遺の最終解析時点で、登録した被験者19例中10例(52.6%)が13サイクル以上の治験薬投与を開始した。最大51サイクルが3例で報告された。治験薬投与の平均サイクル数(標準偏差)は、19.5サイクル(17.19サイクル)であった。総治験期間及び治験薬の総暴露期間の平均値(標準偏差)は、それぞれ1169.4日(363.30日)及び543.5日(482.18日)であった。用量強度は、総1日量の合計を総暴露期間によって除して算出した。相対強度及び相対用量強度の平均値(標準偏差)は、それぞれ154.33 mg/日(77.518mg/日)及び51.44% (25.839%)であった。 各サイクルの開始用量レベルは、治験実施計画書から逸脱した1例(開始用量200 mg)を除き、登録した被験者の大部分に対して開始用量として治験薬300 mgが投与されたことを示している。サイクル5の治験薬投与開始時に、登録した被験者が投与を受けていた用量は、14例中2例(14.3%)が300 mg、4例(28.6%)が200 mg、8例(57.1%)が100 mgであった。サイクル10の治験薬投与開始時に、登録した被験者が投与を受けていた用量は、10例中1例(10.0%)が300 mg、2例(20.0%)が200 mg、7例(70.0%)が100 mgであった。サイクル20の治験薬投与開始時に、登録した被験者が投与を受けていた用量は、7例中1例(14.3%)が300 mg、2例(28.6%)が200 mg、4例(57.1%)が100 mgであった。サイクル30では、被験者4例が治験薬の投与を受けた。用量の内訳は、全例が100 mgであった。 |
Enrollment completed before the 17 March 2019 data cut-off of the original CSR. After completion of the original CSR, some baseline characteristics was changed due to data re-evaluation by the investigator. In 1 subject, the best response during the last platinum regimen was changed from CR to PR. In addition, she had had a metastatic disease in other site (mesentery). Patients were between 44 and 79 years of age. The median age was 62.0 years of age, consistent with ovarian cancer primarily being reported in women over 50 years of age. The mean body weight was 57.47 kg, which was lower than the corresponding mean weight from the overseas Phase 3 study (NOVA study) population (69.69 kg). The most frequent time to progression after the penultimate (next to last) platinum therapy was more than 12 months (14 subjects, 73.7%). All the enrolled subjects had experienced CR (8 subjects, 42.1%) or PR (11 subjects, 57.9%) during the last platinum regimen. All 19 subjects had prior history of myelosuppression. Seventeen of the 19 subjects (89.5%) were reported ECOG performance status as 0, and the rest of them (2 subjects, 10.5%) were reported as 1. The primary tumor site of enrolled subjects was ovarian (10 subjects, 52.6%), primary peritoneal (5 subjects, 26.3%), and fallopian tube (4 subjects, 21.1%). The mean value (standard deviation) of baseline platelet count were 214.1*109/L (58.06*109/L). At the final analysis for the CSR addendum, 10 of the 19 enrolled subjects (52.6%) started 13 or more cycles of the study drug treatment. A maximum of 51 cycles was reported in 3 subjects. The mean (standard deviation) cycle of the study drug treatment was 19.5 (17.19) cycles. The mean (standard deviation) total study duration and overall treatment exposure of the study drug were 1169.4 (363.30) days and 543.5 (482.18) days, respectively. The dose intensity was defined as the sum of the total daily doses ingested divided by overall treatment exposure. The mean (standard deviation) dose intensity and relative dose intensity were 154.33 (77.518) mg/day and 51.44 (25.839) %, respectively. The initial dose level of each cycle shows that most of the enrolled subjects received 300 mg of the study drug as the initial dose except a subject with protocol deviation (200 mg of initial dose). At the beginning of the Cycle 5 of the study drug treatment, 2 of 14 subjects (14.3%) received 300 mg, 4 subjects (28.6%) received 200 mg, and 8 subjects (57.1%) received 100 mg. At the beginning of the Cycle 10 of the study drug treatment, 1 of 10 subjects (10.0%) received 300 mg, 2 subjects (20.0%) received 200 mg, and 7 subjects (70.0%) received 100 mg. At the beginning of the Cycle 20 of the study drug treatment, 1 of 7 subjects (14.3%) received 300 mg, 2 subjects (28.6%) received 200 mg, and 4 subjects (57.1%) received 100 mg. In the Cycle 30 treatment, 4 subjects were administered the study drug; all the subjects received 100 mg. |

Primary Outcome Results; Refer to attached file on this record. Overall, the safety profile was acceptable and consistent with the known safety profile of niraparib and previous experience with niraparib in non-Japanese subjects. The primary endpoint of the incidence of Grade 3 or 4 thrombocytopenia within 30 days was not changed from the data reported in the original CSR. The incidence of Grade 3 or 4 thrombocytopenia within 30 days was 31.6%, which was lower than the expected incidence (46%) from the overseas phase 3 study (NOVA study) subpopulation with baseline body weight of <58 kg, and comparable to the incidence (34.6%) from NOVA study subpopulation with body weight <77 kg or baseline platelet count <150,000/microL, or that (28.9%) from the overall population of NOVA study. All the 19 subjects experienced at least 1 TEAE. The most frequently reported TEAEs in more than 30% of enrolled subjects were nausea (73.7%), platelet count decreased (63.2%), neutrophil count decreased (57.9%), anaemia, vomitting, and decreased appetite (47.4%), white blood cell count decreased and headache (36.8% each), and nasopharyngitis (31.6%). Of 19 enrolled subjects, 10 (52.6%) experienced at worst Grade 3 TEAEs and 4 (21.1%) experienced at worst Grade 4 TEAEs. The common Grade 3 or higher TEAEs were hematologic toxicities associated with myelosuppression. No on-treatment death was reported during the study. Serious TEAEs were reported in 4 subjects (5 events). In the serious TEAEs, the following events were considered related to the study drug: thrombocytopenia, pneumonia, and leukaemia (1 subject each). The serious leukaemia was identified as a AESI and resulted in death. TEAEs leading to study drug dose reduction and study drug dose interruption were reported in 16 subjects each. TEAEs leading to study drug discontinuation were reported in 5 subjects. No new safety signals were identified in this study. |

1. Safety: Number of Participants with Treatment-Emergent Adverse Events (TEAEs) Timeframe; From the day of signing the informed consent form (ICF) until 30 days after last dose of study drug or beginning of subsequent anticancer therapy, whichever comes first (up to 48 months). An AE is defined as any untoward medical occurrence in a participant who has enrolled in a study; it does not necessarily have a causal relationship with this treatment. A treatment-emergent adverse event (TEAE) is defined as an adverse event with an onset that occurs after receiving study drug. 2. Safety: Number of Participants with Grade 3 or Higher TEAEs Timeframe; From the day of signing the ICF until 30 days after last dose of study drug or beginning of subsequent anticancer therapy, whichever comes first (up to 48 months). A severity grade is defined by the NCI-CTCAE Version 4.03. Grade 1 scales as Mild; Grade 2 scales as Moderate; Grade 3 scales as severe or medically significant but not immediately life threatening; Grade 4 scales as life-threatening consequences; and Grade 5 scales as death related to AE. 3. Safety: Number of Participants with Serious Adverse Events (SAEs) Timeframe; From the day of signing the ICF until 30 days after last dose of study drug or beginning of subsequent anticancer therapy, whichever comes first (up to 48 months). An SAE is any AE that results in death, is life-threatening, requires inpatient hospitalization or prolongation of an existing hospitalization, results in significant disability or incapacity, is a congenital anomaly/birth defect or is a medically important event. 4. Safety: Number of Participants with TEAEs Leading to Drug Discontinuation Timeframe; From the day of signing the ICF until 30 days after last dose of study drug or beginning of subsequent anticancer therapy, whichever comes first (up to 48 months). 5. Safety: Number of Participants with TEAEs Leading to Dose Interruption Timeframe; From the day of signing the ICF until 30 days after last dose of study drug or beginning of subsequent anticancer therapy, whichever comes first (up to 48 months). 6. Safety: Number of Participants with TEAEs Leading to Dose Reduction Timeframe; From the day of signing the ICF until 30 days after last dose of study drug or beginning of subsequent anticancer therapy, whichever comes first (up to 48 months). 7. Efficacy: Progression Free Survival (PFS) Timeframe; From first study drug administration until disease progression or death (up to 48 months) PFS is defined as the time in months from the date of first study drug administration to the date of first documentation of progressive disease (PD) or death as assessed by the Investigator per Response Evaluation Criteria in Solid Tumors (RECIST) version 1.1. Per RECIST 1.1, PD is defined as at least a 20% increase in the SoD (Sum of Diameters) of target lesions, taking as a reference the smallest (nadir) SoD since (and including) baseline. In addition to the relative increase of 20%, the SoD must also demonstrate an absolute increase of at least 5 mm. 8. Efficacy: Overall Survival (OS) Timeframe; Up to 48 months OS is defined as the time from the study enrollment to death due to any cause. 9. Efficacy: Overall Response Rate (ORR) Timeframe; Up to 48 months ORR is defined as the proportion of participants achieving Complete Response (CR) or Partial Response (PR) as assessed by the investigator per RECIST (v.1.1). Per RECIST 1.1, CR is defined as disappearance of all target lesions; PR is defined as at least 30% decrease in sum of diameters (SoD) of target lesions. |
