本治験の被験薬はイキサゾミブであり、再発又は難治性の多発性骨髄腫(RRMM)患者を対象に投与される。 本治験はイキサゾミブをレナリドミド及びデキサメタゾンに併用した場合の無増悪生存期間(PFS)、全生存期間(OS)及び安全性をプラセボにレナリドミド及びデキサメタゾンを併用した場合と比較して検討する。 本治験には722例の被験者を組み入れ、以下の2群のうちの1つにランダムに割り付けた。割り付けられた治験薬に関する情報は治験実施中、被験者及び治験担当医師には(医療上緊急でない限り)知らされない: - イキサゾミブ4 mg + レナリドミド + デキサメタゾン - プラセボ(偽薬;被験薬と見た目は同じだが、有効成分を含まない)+ レナミリド + デキサメタゾン すべての被験者は病勢進行又は忍容不能な毒性の発現まで、28日サイクルで投与を受ける。 本多施設共同治験は世界中で実施され、治験期間は約80ヶ月である。被験者は治験期間中に複数回来院し、4週間ごとにPFS、12週間ごとにOSの追跡調査を受ける。 |

722 | ||
/ | 26ヵ国の147施設で合計722例の被験者がランダム化された。これらの被験者のうち、67%は欧州、20%はアジア太平洋(APAC)地域、13%は北米で組み入れられた。 人口統計学的特性は2つの投与レジメン群間で均衡が取れていた。全体として、被験者の大多数は白人であった(85%)。アジア人は9%、黒人/アフリカ系アメリカ人は2%であった。被験者の57%は男性であった。年齢の中央値は66歳(範囲:30~91歳)であった。被験者の48%が65歳以下、37%が65歳超75歳以下、15%が75歳超であった。層別因子に基づくと、被験者の59%が1ラインの前治療歴を有し、41%が2~3ラインの前治療歴を有していた。被験者の70%はプロテアソーム阻害剤の投与歴を有していた。また、被験者の88%がスクリーニング時にISS I期又はII期であった。MMの初期診断から治験薬の初回投与までの期間の中央値は42.8ヵ月(範囲:3~306ヵ月)であった。 被験者集団の70%がプロテアソーム阻害剤の前治療歴を有し、55%が免疫調節剤(IMiD)の前治療歴を有していた。大部分の前レジメンにはコルチコステロイド(98%)が含まれ、大部分にメルファラン[81%、幹細胞移植(SCT)の57%を含む]又はボルテゾミブ(69%)が含まれた。全体として、本試験対象集団の77%が1ライン以上の前治療後に再発し、被験者の11%が1ライン以上の前治療に対して抵抗性を示し、被験者の11%が1ライン以上の前治療後に再発し、かつ1種類以上の前治療に対して抵抗性を示した。前治療薬の最終投与から治験薬の初回投与までの期間の中央値は14.0ヵ月であった。前治療中の病勢進行から治験薬初回投与までの期間の中央値は2.3ヵ月であった。 ベースラインの疾患特性は2つの投与レジメン群間で均衡が取れていた。ベースライン時の最も一般的な骨髄腫の型はIgGサブタイプ(55%)であり、17%でIgAサブタイプが報告された。イキサゾミブレジメン群及びプラセボレジメン群で、それぞれ21%及び17%に細胞遺伝学的高リスク(Del(17)、t(4;14)又はt(14;16)と定義される)が認められた。高リスクの被験者137例のうち97%は中央検査機関で細胞遺伝学的結果が得られ、3%は治験実施医療機関の検査室で細胞遺伝学的結果が得られた。試験集団全体では、69例(10%)がdel(17)異常のみ、又はt(4;14)とt(14;16)の片方又は両方の転座との組み合わせを有する骨髄腫であった。61例(8%)はt(4;14)転座のみを有する骨髄腫、7例(1%)はt(14;16)転座のみを有する骨髄腫であった。 |
A total of 722 participants were randomized at 147 study centers in 26 countries. Of these participants, 67% were enrolled from Europe, 20% were enrolled from the Asia-Pacific (APAC) region, and 13% were enrolled from North America. Demographic characteristics were well balanced between the 2 treatment regimens. Overall, the majority of participants were white (85%); 9% were Asian, and 2% were Black/African American. Fifty-seven percent of the participants were males. The median age was 66 years, with a range of 30 to 91 years; 48% of participants were =<65 years, 37% were >65 to =<75 years, and 15% were >75 years. On the basis of the stratification factors, 59% of participants had 1 prior line of therapy and 41% had 2 or 3 prior lines of therapy; 70% of participants were proteasome inhibitor-exposed; and 88% of participants had an ISS Stage of I or II at screening. The median time since initial diagnosis of MM and the first dose of study drug was 42.8 months (range, 3 to 306 months). While 70% of the participant population had been exposed to proteasome inhibitor therapy, 55% had been exposed to immunomodulatory drug (IMiD) therapy. Most prior regimens contained corticosteroids (98%), and the majority contained melphalan (81%, including 57% for stem cell transplant [SCT]) or bortezomib (69%). Overall, 77% of the study population had relapsed from at least 1 previous treatment, 11% of participants were refractory to at least 1 previous treatment, and 11% of participants had both relapsed from and were refractory to at least 1 previous treatment. The median interval since the last dose of prior therapy to the first dose of study drug was 14.0 months; the median interval since disease progression on prior therapy to the first dose of study drug was 2.3 months. Baseline disease characteristics were well balanced between the 2 treatment regimens. IgG subtype (55%) was the most common type of myeloma at baseline, with IgA subtype reported in 17%. In the ixazomib and placebo regimens, high-risk cytogenetics (defined as del[17], t[4;14], or t[14;16]) were identified in 21% and 17% of participants, respectively; 97% of the 137 high-risk participants had cytogenetic results from the central laboratory, while 3% had cytogenetic results from the local laboratory. In the overall study population, 69 participants (10%) had myeloma harboring del(17) abnormality alone or in combination with either or both of the translocations t(4;14) and t(14;16); 61 participants (8%) had myeloma harboring t(4;14) translocation alone, and 7 participants (1%) had t(14;16) translocation alone. |

All 722 participants received LenDex and were randomized to receive, in addition, ixazomib or placebo. For ease of discussion within the text, the 2 treatment regimens are referred to by the randomized drug name only: the ixazomib + LenDex regimen is referred to as the ixazomib regimen; the placebo + LenDex regimen is referred to as the placebo regimen. Although the comparator regimen is referred to as the placebo regimen, it is important to note that the safety and efficacy of ixazomib plus LenDex are being compared to the safety and efficacy of placebo plus LenDex. Two placebo regimen participants identified in the CSR (data cutoff date: 30 October 2014) and 1 additional placebo regimen patient identified in Addendum 1 (data cutoff date: 12 July 2015) were erroneously given ixazomib regimen kits at some cycles during treatment; these 3 participants were excluded from the safety population of the placebo regimen and included in the safety population of the ixazomib regimen. |
/ | イキサゾミブの毒性プロファイルは十分に解明済みかつ忍容性も良好で管理可能であり、他のプロテアソーム阻害剤の毒性プロファイルに関する公表済みのデータと比較して良好である。今回の最終解析による安全性データから、イキサゾミブの週1回経口投与をLenDexのバックグラウンドレジメンと併用しても、バックグラウンドレジメンの安全性プロファイルが大幅に変化することはないことが引き続き示された。 2020年9月28日(最終解析のデータカットオフ日)時点: グレード3のTEAEを発現した被験者の割合は、イキサゾミブレジメン群の方がプラセボレジメン群よりも高かった(54% vs 48%)。被験者の10%以上に発現した基本語別のグレード3のTEAEとしては、好中球減少症、貧血及び肺炎が各レジメン群で、また下痢がイキサゾミブレジメン群で認められた。基本語別では、グレード3の血小板減少症(9% vs 4%)及び下痢(10% vs 3%)の発現率はイキサゾミブレジメン群の方がプラセボレジメン群よりも高かった。グレード4のTEAEを発現した被験者の割合はレジメン間で同程度であった(イキサゾミブレジメン群20%、プラセボレジメン群17%)。 2020年9月28日のデータベース固定時点で、試験中の死亡は合計51件(イキサゾミブレジメン群21件、プラセボレジメン群30件)が報告された。イキサゾミブレジメン群の死亡12件及びプラセボレジメン群の死亡15件は治験対象疾患に関連するものであった。残りのイキサゾミブレジメン群の死亡9件及びプラセボレジメン群の死亡15件については、治験対象疾患と関連のないTEAEによるものであった。試験中に死亡した51件のうち、イキサゾミブレジメン群の4件及びプラセボレジメン群の3件は治験薬と関連ありと判定された。 試験全体を通じて、重篤な有害事象(SAE)が発現した被験者の割合はレジメン群間で同程度であった(イキサゾミブレジメン群57%、プラセボレジメン群56%)。器官別大分類では、感染症及び寄生虫症が2つのレジメン群で最も高頻度に認められたSAEであった(イキサゾミブレジメン群28%、プラセボレジメン群30%)。最も高頻度に認められた基本語別のSAEは、いずれのレジメン群とも肺炎(各レジメン群13%)であった。SAEが発現した被験者の割合(基本語別)はレジメン群間で同程度であった。 臨床検査値、バイタルサイン、心電図及びEastern Cooperative Oncology Group全身状態スコアの差は、概して臨床的に意味のあるものではなかった。 2022年6月29日時点(最終解析日以降): 全体で、治験薬を投与中の27例(96%)に1件以上のTEAEが報告された。イキサゾミブレジメン群の3例(20%)が投与期間中に治験薬の投与を中止した。イキサゾミブレジメン群の1例(7%)に治験薬と関連のある重篤な有害事象(SAE)が発現した。イキサゾミブレジメン群の1例(7%)が投与期間中に死亡した。 投与期間中において合計28例のうち2例以上に発現した主なTEAEを示す。全体として、被験者の20%以上に発現した主なTEAEは下痢(10例[36%])、末梢性感覚ニューロパチー(9例[32%])、不眠症、好中球減少症及び関節痛(各7例[25%])、貧血及び背部痛(6例[21%])であった。イキサゾミブレジメン群の20%以上に発現した主なTEAEは下痢(6例[40%])、貧血(5例[33%])、末梢性感覚ニューロパチー、不眠症、好中球減少症及び頭痛(各4例[27%])、関節痛及び背部痛(各3例[20%])であった。プラセボレジメン群の20%以上に発現した主なTEAEは、末梢性感覚ニューロパチー(5例[38%])、関節痛及び下痢(各4例[31%])、不眠症、好中球減少症、背部痛、疲労及び末梢性浮腫(各3例[23%])であった。 前回のデータカットオフ以降に17例が死亡した(イキサゾミブレジメン群10例、プラセボレジメン群7例)。イキサゾミブレジメン群の1例が投与期間中に死亡した。当該被験者は最終投与から18日後のサイクル101のDay 26(Study Day 2974, 2021年11月1日)に発現したCOVID-19肺炎の治療中に死亡した。この死亡は治験担当医師により治験薬と関連なしと判断された。プラセボレジメン群では投与期間中の死亡はなかった。17例中16例は追跡期間中に死亡した。17例の死亡のうち11例は疾患進行によるものであった(イキサゾミブレジメン群7例、プラセボレジメン群4例)。他の5例の死亡は治験薬と関連なしと判断された。 最終被験者の最終来院(LPLV)の時点で、8例(29%)(イキサゾミブレジメン群5例[33%]、プラセボレジメン群3例[23%])に計10件のSAEが発現していた。LPLV時点で、イキサゾミブレジメン群の1例(7%)において、結腸腺癌が治験薬と関連のあるSAEとして報告された。 |
Ixazomib has a well-characterized, tolerated, and manageable toxicity profile that compares favorably to published data of toxicity profiles of other proteasome inhibitors. The safety data from this FA continue to demonstrate that oral ixazomib administered once weekly in combination with a background regimen of LenDex did not substantially alter the safety profile of the background regimen. As of 28 September 2020 (the date of data cutoff for the final analysis): The percentage of participants with Grade 3 TEAEs was higher in the ixazomib regimen than the placebo regimen (54% vs 48%). Grade 3 TEAEs by Preferred Term that occurred in >=10% of participants were neutropenia, anaemia, and pneumonia in each regimen; and diarrhoea in the ixazomib regimen. By Preferred Term, Grade 3 thrombocytopenia (9% vs 4%) and diarrhoea (10% vs 3%) occurred more frequently in the ixazomib regimen than in the placebo regimen. The percentage of participants with Grade 4 TEAEs was similar between regimens (20% ixazomib regimen, 17% placebo regimen). A total of 51 on-study deaths (21 ixazomib regimen, 30 placebo regimen) were reported at the time of the database lock on 28 September 2020. Twelve deaths in the ixazomib regimen and 15 deaths in the placebo regimen were related to the disease under study. Nine and 15 other participants in the ixazomib and placebo regimen, respectively, died due to TEAEs that were not related to the disease under study. Of the 51 on-study deaths, 4 in the ixazomib regimen and 3 in the placebo regimen were assessed as related to study drug. Throughout the study, the percentage of participants with serious adverse events (SAEs) was similar between regimens (57% ixazomib regimen, 56% placebo regimen). By System Organ Class, infections and infestations were the most common SAEs in both regimens (28% ixazomib regimen, 30% placebo regimen); the most common SAE by Preferred Term was pneumonia in each regimen (13% in each regimen). The percentage of participants who experienced SAEs by Preferred Term was similar between regimens. Differences in clinical laboratory results, vital sign, ECG, and Eastern Cooperative Oncology Group performance status scores were generally not clinically significant. As of 29 June 2022 (after the date of the final analysis): Overall, a total of 27 (96%) patients on treatment reported at least 1 TEAE. Three patients (20%) in the ixazomib regimen discontinued any study drug on treatment. One patient (7%) in the ixazomib regimen experienced a treatment-related serious adverse event (SAE). One patient (7%) died on treatment in the ixazomib regimen. The most common TEAEs in more than 2 patients of the 28 total on treatment are presented. Overall, the most common TEAEs in >=20% of patients were diarrhoea (10 patients [36%]); peripheral sensory neuropathy (9 patients [32%]); insomnia, neutropenia, and arthralgia (7 patients [25%] each); and anaemia and back pain (6 patients [21%]). The most common TEAEs in >=20% of patients in the ixazomib regimen were diarrhoea (6 patients [40%]); anaemia (5 patients [33%]); peripheral sensory neuropathy, insomnia, neutropenia, and headache (4 patients [27%] each); and arthralgia and backpain (3 patients [20%] each). The most common TEAEs in >=20% of patients in the placebo regimen were peripheral sensory neuropathy (5 patients [38%]), arthralgia and diarrhoea (4 patients [31%] each); and insomnia, neutropenia, back pain, fatigue, and oedema peripheral (3 patients [23%] each). Seventeen patients died since the last data cutoff date (10 patients in the ixazomib regimen and 7 patients in the placebo regimen); 1 patient in the ixazomib regimen died on treatment. That patient died on treatment of COVID-19 pneumonia, which occurred on Day 26 of Cycle 101 (Study Day 2974, 01 November 2021), 18 days from the last treatment dose. This death was considered not related to study treatment by the investigator. In the placebo regimen, none of the deaths were on treatment. Sixteen of the 17 deaths occurred during the follow-up period. Eleven of the 17 deaths were due to disease progression (7 deaths in the ixazomib regimen and 4 deaths in the placebo regimen); the other 5 deaths were assessed as not related to study treatment. Eight patients (29%) (5 patients [33%] in the ixazomib regimen and 3 patients [23%] in the placebo regimen) had a total of 10 SAEs on treatment as of Last Patient Last Visit (LPLV). One patient (7%) in the ixazomib regimen reported a treatment-related SAE of adenocarcinoma of colon as of LPLV. |

-Progression-free survival: At this first analysis (data cutoff date: 30 October 2014), with median durations of follow-up of 14.8 months and 14.6 months in the ixazomib and placebo regimens, respectively, 129 ixazomib regimen participants (36%) and 157 placebo regimen participants (43%) had experienced PD or death (286 events) as assessed by the IRC. The primary endpoint of IRC-assessed PFS was significantly improved by 35% with the ixazomib+LenDex regimen compared to the placebo+LenDex regimen (HR=0.742; p=0.012). The difference in PFS was also clinically meaningful, with an improvement in median PFS of approximately 6 months (20.6 months vs 14.7 months) for the ixazomib regimen versus the placebo regimen. All sensitivity analyses of IRC-assessed PFS were consistent with the primary analysis, with HRs ranging from 0.713 to 0.748, indicating improvements with the ixazomib regimen of 34% to 40% over the placebo regimen. Median differences in IRC-assessed PFS between the ixazomib regimen and the placebo regimen ranged from 5.1 to 6.5 months, indicating consistent clinically meaningful results. The difference in PFS was corroborated by a clinically meaningful improvement in TTP. Among the 114 ixazomib regimen participants (32%) and 145 placebo regimen participants (40%) who experienced documented PD, the median time to progression by IRC was 21.4 months and 15.7 months, respectively (HR=0.712; p=0.007). PFS was evaluated relative to baseline stratification factors, demographic characteristics, disease characteristics, and number and types of prior therapy. The HRs were <1 across the prespecified subgroups, indicating consistent reduction in the risk of progression or death in the ixazomib regimen compared with the placebo regimen. |
副次的評価項目の解析結果 / Secondary Outcome Measures | -全生存期間:C16010の治験総括報告書補遺2(データカットオフ日:2020年9月28日)の主要解析ではIntent-to-treat(ITT)集団におけるOSを対象とした。最終解析のデータカットオフ時点(2020年9月28日)で、OSの最終解析に必要な規定死亡件数の99%超(484/486)が報告された。OSの中央値はイキサゾミブレジメン群が53.6ヵ月、プラセボレジメン群が51.6ヵ月であった。イキサゾミブレジメン群でわずかな改善傾向が認められたものの(ハザード比[HR]=0.939、95%CI:0.784, 1.125、p=0.495)、統計学的に有意ではなかった。 -del(17p)を有する被験者の全生存期間:ITT集団におけるOSは統計学的に有意ではなかったため、2番目の重要な副次評価項目であるdel(17p)を有する高リスクの被験者におけるOSについては正式な検定を行わなかった。2020年9月28日の解析時点でこれらの被験者のうち、イキサゾミブレジメン群の被験者36例中28例(78%)、プラセボレジメン群の被験者33例中25例(76%)が死亡した。del(17p)を有する被験者におけるOSの中央値は、イキサゾミブレジメン群が42.2ヵ月、プラセボレジメン群が29.4ヵ月であった(HR=0.916、95%CI:0.516, 1.626、p=0.764)。del(17p)を有する被験者では、イキサゾミブレジメン群の方がOSに関して良好な傾向を示しており、これはIA 1(データカットオフ日:2014年10月30日)及びIA 2(データカットオフ日:2015年7月12日)の解析結果と合致していた。 -高リスクの細胞遺伝学的所見を有する被験者におけるOS:del(17p)、t(4;14)又はt(14;16)を含む高リスクの細胞遺伝学的異常を有する被験者のうち(N=137、イキサゾミブレジメン群75例、プラセボレジメン群62例)、イキサゾミブレジメンを受けた被験者ではプラセボレジメンを受けた被験者と比較して死亡リスクが13%低下した(HR=0.870、95%CI:0.580, 1.305、p=0.500)。またOSの中央値はイキサゾミブレジメン群が46.9ヵ月、プラセボレジメン群が30.9ヵ月であった。del(17p)、t(4;14)、t(14;16)及び/又は1q21増幅を有するさらに高リスクの被験者群(N=309、イキサゾミブレジメン群155例、プラセボレジメン群154例)についても、OSの評価を実施した。これらの被験者のうち、イキサゾミブレジメン群ではプラセボレジメン群と比較して死亡リスクが14%低下し(HR=0.862、95%CI:0.660, 1.124、p=0.273)、OSの中央値はそれぞれ44.6ヵ月と33.4ヵ月であった。 -生活の質(QOL):主要解析(データカットオフ日:2014年10月30日)の結果と同様、EORTC QLQ-C30、EORTC QLQ-MY20、EQ-5D-3L及びEQ VASで測定したQOLは、いずれのレジメン群でもおおむね維持されていた。EORTC QLQ-C30の全般的健康状態/QOLサブスケールスコアの経時的変化は、2つのレジメン群間で一貫して同程度であった。初期のサイクルでは、疼痛スコアの平均値はいずれのレジメン群でもわずかに改善したが、便秘スコアの平均値はいずれのレジメン群でも悪化した。下痢の症状はいずれのレジメン群とも一貫してベースラインから経時的に悪化し、イキサゾミブレジメン群では複数のサイクルでわずかに悪化する傾向が認められた。治療の副作用を測定したサブスケールに関する結果は、イキサゾミブレジメン群とプラセボレジメン群で類似していた。イキサゾミブとLenDexとの併用は、健康関連QOLに悪影響を及ぼさないと考えられた。 | -Overall survival: The primary analysis of C16010 CSR Addendum 2 (data cutoff date: 28 September 2020) was OS in the Intent-to-treat (ITT) population. As of the data cutoff for the final analysis (28 September 2020), >99% (484/486) of prespecified deaths required for final OS analysis were reported. The median OS was 53.6 months for participants in the ixazomib regimen and 51.6 months for participants in the placebo regimen; a slight trend in favor of the ixazomib regimen was seen (hazard ratio [HR] =0.939; 95% CI: 0.784, 1.125; p = 0.495), which was not statistically significant. -Overall survival in participants harboring del(17p): Because OS in the ITT population did not attain statistical significance, the second key secondary endpoint, OS in high-risk participants harboring del(17p), was not formally tested. Among these participants as of the 28 September 2020 analysis, 28 of 36 (78%) ixazomib regimen participants and 25 of 33 (76%) placebo regimen participants had died. The median OS among participants harboring del(17p) was 42.2 months for participants in the ixazomib regimen and 29.4 months for participants in the placebo regimen (HR=0.916; 95% CI: 0.516, 1.626; p=0.764). The positive OS trend in favor of the ixazomib regimen in participants with del(17p) is consistent with the analyses of IA1 (data cutoff date: 30 October 2014) and IA2 (data cutoff date: 12 July 2015). -OS in participants with high-risk cytogenetics: Among participants with high-risk cytogenetic abnormalities including del(17p), t(4;14), or t(14;16) (N=137; 75 ixazomib regimen, 62 placebo regimen), a 13% reduction in the risk of death was observed for participants who received the ixazomib regimen compared with participants who received the placebo regimen (HR=0.870; 95% CI: 0.580, 1.305; p=0.500); the median OS was 46.9 months with the ixazomib regimen and 30.9 months with the placebo regimen. OS was also assessed in an expanded high-risk group of participants with del(17p), t(4;14), t(14;16), and/or amplification 1q21 (N=309; 155 ixazomib regimen, 154 placebo regimen). Among these participants, a 14% reduction in the risk of death was observed for participants in the ixazomib regimen compared with participants in the placebo regimen (HR=0.862; 95% CI: 0.660, 1.124; p=0.273), with median OS of 44.6 months and 33.4 months, respectively. -Quality of life (QOL): Consistent with the results from the primary analysis (data cutoff date: 30 October 2014), QOL, as measured by the EORTC QLQ-C30, EORTC QLQ-MY20, EQ-5D-3L, and EQ VAS, was generally maintained with both treatment regimens. Global Health Status/QOL subscale scores on the EORTC QLQ-C30 over time were consistently similar between the 2 regimens. Mean Pain scores improved slightly in both regimens and mean scores for Constipation worsened in both regimens at earlier cycles; symptoms of Diarrhoea consistently worsened from baseline over time in both regimens, with the ixazomib regimen trending slightly worse at a number of cycles. The results of the subscale measuring Side Effects of Treatment were similar among participants in the ixazomib regimen and placebo regimens. The addition of ixazomib to LenDex did not appear to have a negative impact on health-related QOL. |

本治験の被験薬はイキサゾミブであり、再発又は難治性の多発性骨髄腫(RRMM)患者を対象に投与される。 本治験はイキサゾミブをレナリドミド及びデキサメタゾンに併用した場合の無増悪生存期間(PFS)、全生存期間(OS)及び安全性をプラセボにレナリドミド及びデキサメタゾンを併用した場合と比較して検討する。 本治験には722例の被験者を組み入れ、以下の2群のうちの1つにランダムに割り付けた。割り付けられた治験薬に関する情報は治験実施中、被験者及び治験担当医師には(医療上緊急でない限り)知らされない: - イキサゾミブ4 mg + レナリドミド + デキサメタゾン - プラセボ(偽薬;被験薬と見た目は同じだが、有効成分を含まない)+ レナミリド + デキサメタゾン すべての被験者は病勢進行又は忍容不能な毒性の発現まで、28日サイクルで投与を受ける。 本多施設共同治験は世界中で実施され、治験期間は約80ヶ月である。被験者は治験期間中に複数回来院し、4週間ごとにPFS、12週間ごとにOSの追跡調査を受ける。 | The drug being tested in this study is called Ixazomib. Ixazomib is being tested to treat people who have relapsed and/or refractory multiple myeloma (RRMM). This study will look at progression free survival (PFS), overall survival (OS) and safety in participants who take ixazomib in addition to lenalidomide and dexamethasone compared to placebo in addition to lenalidomide and dexamethasone. The study enrolled 722 patients. Participants were randomly assigned (by chance, like flipping a coin) to one of the two treatment groups - which will remain undisclosed to the patient and study doctor during the study (unless there is an urgent medical need): - Ixazomib 4 mg + lenalidomide + dexamethasone - Placebo (dummy inactive pill) - this is a tablet that looks like the study drug but has no active ingredient + lenalidomide + dexamethasone All participants will receive treatment in 28 day cycles until disease progression or unacceptable toxicity. This multi-center trial will be conducted worldwide. The overall time to participate in this study is approximately 80 months. Participants will make multiple visits to the clinic, and will be contacted every 4 weeks for PFS and every 12 weeks for OS. | ||

Progression-free survival (PFS) as Assessed by the Independent Review Committee (IRC) Timeframe: From date of randomization until disease progression or death up to approx. 27 months (approximate median follow-up 15 months) PFS is defined as the time from the date of randomization to the date of first documentation of disease progression (PD) or death due to any cause, whichever occurs first. Response including PD was assessed by IRC using the International Myeloma Working Group (IMWG) response criteria. IRC判定による無増悪期間(TTP) 評価期間:病勢進行まで4週間ごと(各サイクルの1日目)、最長約27ヵ月(追跡調査期間の中央値は約15ヵ月) TTPは治験治療薬の初回投与からIRCによって評価されたIMWGの基準に基づくPDが初めて確認された日までの期間とする。 7. 有害事象及び重篤な有害事象が発現した被験者の割合 評価期間:同意取得時から治験治療薬の最終投与30日後まで(最長約115ヵ月) ECOG performanceスコア、臨床検査値、バイタルサイン測定値及び有害事象を収集し、治験全体を通して治験治療の安全性を評価する。有害事象とは、治験治療薬を投与された被験者に生じたあらゆる好ましくない医療上の出来事であり、治験治療薬との因果関係の有無は問わない。したがって、有害事象とは、治験治療薬投与と時間的に関連する好ましくない、あるいは意図しない徴候(臨床検査値の異常変動を含む)、症状又は病気のことであり、治験治療薬との因果関係の有無は問わない。重篤な有害事象は以下のいずれかをもたらす又は他の何らかの理由で重篤とみなされる有害事象:死に至るもの、入院又は入院期間の延長が必要となるもの、生命を脅かすもの(死に至る危険性)、永続的又は顕著な障害・機能不全に陥るもの、先天異常を来すもの、その他の医学的に重要な状態。 8. 疼痛緩和が得られた被験者数のベースラインからの変化 評価期間:ベースライン及び投与終了時(最長約38ヵ月) 疼痛緩和は、連続2回の評価で、鎮痛剤の使用量(経口モルヒネ当量)を増加させずにBrief Pain Inventory-Short Form (BPI-SF)のWorst Painスコアが直近の24時間でベースラインから2ポイント又は30%以上低下することと定義する。 BPI-SFは疼痛の重症度(「最も強い痛み」、「最も弱い痛み」、「平均した痛み」、「今感じている痛み」)、疼痛の部位、疼痛緩和のための投薬、疼痛によるさまざまな日常生活動作(全般的活動、気分、歩行、通常の仕事、対人関係、睡眠、生活を楽しむことを含む)への支障を調査するための15項目からなる。疼痛の重症度の各項目は0(痛みなし)~10(これ以上の痛みは考えられない)で評価する。 9. European Organization for Research and Treatment of Cancer (EORTC) Core Quality of Life (QOL) Questionnaire (EORTC-QLQ-C30)のベースラインからの変化 評価期間:ベースライン、投与終了時及び追跡調査(最長約97ヵ月) EORTC-QLQ-C30はがん患者を対象に全般的な生活の質(QOL)を評価する30の質問からなる評価ツールである。この質問票は以下の15のドメインで構成されている:全般的健康状態の1 尺度、機能の5 尺度(身体機能、役割機能、情緒機能、認知機能、社会機能)、9つの症状スケール/項目(疲労感、悪心・嘔吐、疼痛、呼吸困難、不眠、食欲不振、便秘、下痢、経済的困難)。EORTC-QLQ-C30の全般的な健康状態/QOLスケールは0から100の範囲でスコア化され、点数が高いほどよりよい健康状態/QOLを示す。ベースラインからのマイナスの変化はQOL又は機能の低下を示し、プラスの変化は改善を示す。スコアを0~100のスケールに線形変換する。全般的及び機能的ドメインのスコアが高いほど、生活の質又は機能が高いことを示す。症状スケールのスコアが高いほど、症状又は問題のレベルが高いことを示す。 10. 1. Overall survival (OS) Timeframe: From date of randomization until death (up to approx. 97 months) OS is defined as the time from the date of randomization to the date of death. Participants without documentation of death at the time of the analysis were censored at the date when they were last known to be alive. 2. OS in High-Risk Participants Carrying Deletion 17 [Del(17)] Timeframe: From the time of screening until disease progression and thereafter every 12 weeks until death or study termination (up to approx. 97 months) The high-risk participants whose myeloma carried del(17) subgroup was defined as the cases reported as positive for del(17) by the central laboratory combined with those cases that lacked a central laboratory result but with known del(17) by local laboratory. Data is only reported high-risk participants with Del(17). 3. Overall Response Rate (ORR) as Assessed by the IRC Timeframe: Day 1 of each cycle (every 4 weeks) until disease progression up to approx. 27 months (approx. median follow-up 15 months) ORR was defined as the percentage of participants with Complete Response (CR) including stringent complete response (sCR), very good partial response (VGPR) and Partial Response (PR) assessed by the IRC using IMWG criteria. 4. Percentage of Participants With CR and VGPR as Assessed by the IRC Timeframe: Day 1 of each cycle (every 4 weeks) until disease progression up to approx. 27 months (approx. median follow-up 15 months) Response was assessed by the IRC using IMWG Criteria. CR is defined as negative immunofixation on the serum and urine and; disappearance of any soft tissue plasmacytomas and; <5% plasma cells in bone marrow. VGPR is defined as Serum and urine M-protein detectable by immunofixation but not on electrophoresis or 90% or greater reduction in serum M-protein plus urine M-protein level <100 mg per 24 hours. 5. Duration of Response (DOR) Timeframe: Day 1 of each cycle (every 4 weeks) until disease progression up to approx. 38 months DOR was measured as the time in months from the date of first documentation of a confirmed response of PR or better (CR [including sCR] + PR+ VGPR) to the date of the first documented PD among participants who responded to the treatment. Response was assessed by the investigator using IMWG Criteria. 6. Time to Progression (TTP) as Assessed by the IRC Timeframe: Day 1 of each cycle (every 4 weeks) until disease progression up to approx. 27 months (approx. median follow-up 15 months) TTP was measured as the time in months from the first dose of study treatment to the date of the first documented PD as assessed by the IRC using IMWG criteria. 7. Number of Participants With Adverse Events (AEs) and Serious Adverse Events (SAEs) Timeframe: From the date of signing of the informed consent form through 30 days after the last dose of study drug up to approx. 115 months ECOG performance score, laboratory values, vital sign measurements and reported AEs were collected and assessed to evaluate the safety of therapy throughout the study. An AE is defined as any untoward medical occurrence in a clinical investigation participant administered a drug; it does not necessarily have to have a causal relationship with this treatment. An AE can therefore be any unfavorable and unintended sign (example, a clinically significant abnormal laboratory finding), symptom, or disease temporally associated with the use of a drug, whether or not it is considered related to the drug. A SAE is an AE resulting in any of the following outcomes or deemed significant for any other reason: death; initial or prolonged inpatient hospitalization; life-threatening experience (immediate risk of dying); persistent or significant disability/incapacity; or congenital anomaly; or a medically important event. 8. Number of Participants with Change From Baseline in Pain Response Timeframe: Baseline and end of treatment (EOT) (up to approx. 38 months) Pain response was defined as 30% reduction from Baseline in Brief Pain Inventory-Short Form (BPI-SF) worst pain score over the last 24 hours without an increase in analgesic (oral morphine equivalents) use at 2 consecutive evaluations. The BPI-SF contains 15 items designed to capture the pain severity ("worst," "least," "average," and "now" [current pain]), pain location, medication to relieve the pain, and the interference of pain with various daily activities including general activity, mood, walking activity, normal work, relations with other people, sleep, and enjoyment of life. The pain severity items are rated on a 0 to 10 scale where: 0=no pain and 10=pain as bad as you can imagine and averaged for a total score of 0 (best) to 10 (Worst). 9. Change From Baseline in the European Organization for Research and Treatment of Cancer (EORTC) Core Quality of Life (QOL) Questionnaire (EORTC-QLQ-C30) Timeframe: Baseline, EOT and follow-up (up to approx. 97 months) The EORTC-QLQ-C30 is a 30-question tool used to assess the overall quality of life in cancer participants. It consists of 15 domains: 1 global health status (GHS) scale, 5 functional scales (Physical, Role, Cognitive, Emotional, Social), and 9 symptom scales/items (Fatigue, Nausea and Vomiting, Pain, Dyspnea, Sleep Disturbance, Appetite Loss, Constipation, Diarrhea, Financial Impact). The EORTC-QLQ-C30 Global Health Status/QOL Scale is scored between 0 and 100, where higher scores indicate better Global Health Status/QOL. Negative changes from baseline indicate deterioration in QOL or functioning and positive changes indicate improvement. Scores are linearly transformed to a 0-100 scale. High scores for the global and functional domains indicate higher quality of life or functioning. Higher scores on the symptom scales represent higher levels of symptomatology or problems. 10. Change From Baseline in the EORTC Quality of Life Questionnaire Multiple Myeloma Module (QLQ-MY-20) Timeframe: Baseline, EOT and follow-up (up to approx. 97 months) The EORTC-QLQ-MY-20 is a patient-completed, 20-question quality of life questionnaire that has 4 independent subscales, 2 functional subscales (body image, future perspective), and 2 symptoms scales (disease symptoms and side-effects of treatment). The participant answers questions about their health during the past week using a 4-point scale where 1=Not at All to 4=Very Much. A negative change from Baseline indicates improvement. Higher scores on the symptom scales (e.g. Disease Symptoms, Side Effects of Treatment) represent higher levels of symptomatology or problems. High scores for Body Image and Future Perspective represent better quality of life or functioning. 11. OS in High-Risk Participants Timeframe: From the time of screening until disease progression and thereafter every 12 weeks until death or study termination (up to approx. 97 months) High-risk participants are defined as participants carrying cytogenic abnormalities: del(17), translocation t(4;14), or t(14;16) as reported by the central laboratory combined with those cases that lacked a central laboratory result but with known del (17), t(4;14), or t(14;16) by local laboratory. Cytogenetic abnormalities of del(13) and +1q are no longer considered to be high-risk abnormalities and are not included in the analysis. Data is only reported for high-risk participants. 12. PFS in High-Risk Participants Timeframe: From date of randomization until disease progression or death up to approx. 38 months. (approx. median follow-up 15 months) 13. Pharmacokinetic Parameters (Including Cmax, AUC and Tmax) of Ixazomib Timeframe: Days 1 and 14 of Cycles 1 and 2. Day 1 of Cycles 3 to 10 14. ORR in Participants Defined by Polymorphism Timeframe: Day 1 of each cycle (every 4 weeks) until disease progression up to approx. 27 months (approx. median follow-up 15 months) Data is reported for percentage of participants defined by polymorphism defined by polymorphisms in proteasome genes, such as polymorphism P11A in PSMB1 gene. |
