1. 本治験の適格性評価の手順を開始する前に、ICF に署名及び同意日を記入している。ベースライン時の腫瘍組織の要件を満たすため、すべての被験者から組織スクリーニングに対する同意を別途取得する。 2. 18 歳以上の男性又は女性(被験者の法定同意年齢が18 歳超の場合、各国の規制要件に従う)。 3. 以下の局所進行切除不能又は転移性癌(手術又は放射線照射によって治癒不能)患者: 3-1. 皮膚又は末端黒子型悪性黒色腫 a. 皮膚又は末端黒子型悪性黒色腫が組織学的又は細胞学的に確定診断されている患者 b. 1 レジメン以上の抗PD-1 抗体又は抗PD-L1 抗体を含む治療を受けた後の病勢進行。BRAFm悪性黒色腫患者の場合、BRAF/MEK 阻害薬による治療が施行され病勢進行が認められたことも組み入れの条件とする。 又は 3-2. 頭頸部扁平上皮癌 c. ヒトパピローマウイルス(human papillomavirus: HPV)陽性又は陰性のSCCHN。腫瘍の原発部位が口腔、中咽頭、下咽頭、又は喉頭であることを組み入れの条件とする。上咽頭から発生した腫瘍は除外する。 d. PBC 後の病勢進行。抗PD-1 療法の併用有無は問わない。 又は 3-3. 胃癌又は胃食道接合部癌 e. ASCO-CAPガイドラインの分類に基づき、治験実施医療機関の検査機関での評価によりHER2 発現陰性(免疫組織化学[immunohistochemistry: IHC]0/1+又はIHC 2+/in situ ハイブリダイゼーション陰性)であることが組み入れ前に腫瘍組織より確定されている。 f. PBC 後の病勢進行。抗PD-1 療法の併用有無は問わない。 4. CT 又はMRI で測定可能な病変(RECIST v1.1 に基づく治験責任医師の判定による)を1 つ以上有する。 5. 十分な量の治験治療前の腫瘍組織検体を提供する。治験治療前の腫瘍組織としては、以下のいずれかが提供されればよい。 a. 直近の抗がん剤レジメンによる治療中又は治療後に確認された病勢進行以降かつ組織ICF への署名前に実施された生検で採取された組織 又は b. 放射線照射歴がなく検体採取に適した1 つ以上の病変から、治験治療前に採取された腫瘍組織 治験依頼者との協議及び合意後に、治験治療前の腫瘍組織の要件を免除する可能性がある。 6. スクリーニング時のECOG PSが0 又は1 である。 |
1. Sign and date the ICF, prior to the start of any study-specific qualification procedures. A separate tissue screening consent will be obtained from all subjects to meet the baseline tumor tissue requirement. 2. Is a male or female aged >=18 years (follow local regulatory requirements if the legal age of consent for study participation is >18 years old). 3. Has locally advanced unresectable or metastatic disease (not curable by surgery or radiation) as follows: 3-1. Cutaneous or acral melanoma a. Histologically or cytologically confirmed cutaneous or acral melanoma b. Disease progression after having received >=1 prior line of anti-PD-1 or anti-PD-L1 based therapy. If the subject had BRAFm melanoma, they must have had disease progression on BRAF/MEK inhibitor therapy as well. OR 3-2. Squamous cell carcinomas of the head and neck c. Squamous cell carcinoma of the head and neck that is human papillomavirus (HPV) positive or negative. Primary tumor site must have arisen initially from the oral cavity, oropharynx, hypopharynx, or larynx. Tumors arising from the nasopharynx are excluded. d. Disease progression after having received treatment with a PBC regimen with or without anti-PD-1 therapy OR 3-3. Gastric or GEJ adenocarcinoma e. Tumor tissue must be confirmed as negative for HER2 expression (immunohistochemistry [IHC] 0/1+ or IHC 2+/in situ hybridization negative) as classified by ASCO-CAP guidelines and determined prior to enrollment by assessment in a local laboratory that is Clinical Laboratory Improvement Amendments certified (US sites) or accredited based on specific country regulations. f. Disease progression after having received treatment with a previous PBC with or without anti-PD-1 therapy. 4. Has >=1 measurable lesion on CT or MRI as per RECIST v1.1 by investigator assessment. 5. Provides a pretreatment tumor tissue sample of sufficient quantity, as defined in the Study Laboratory Manual. The pretreatment tumor tissue can be provided as either a. Tissue collected from a biopsy performed since progression while on or after treatment with the most recent cancer therapy regimen and prior to signing of the tissue ICF OR b. Pretreatment tumor biopsy from >=1 lesion not previously irradiated and amenable to sampling The pretreatment tissue requirement may be waived after discussion and agreement with the Sponsor. 6. Has Eastern Cooperative Oncology Group performance status (ECOG PS) of 0 or 1 at screening. |

1. ASCO-CAP ガイドラインの分類に基づき、治験実施医療機関の検査機関での評価によりHER2 陽性胃癌であると組み入れ前に判定されている。 2. 上咽頭癌を有する。 3. 粘膜又はぶどう膜悪性黒色腫を有する。 4. 間質性肺疾患(interstitial lung disease: ILD、肺線維症又は放射線性肺臓炎を含む)の既往歴がある、ILD を合併している、又はスクリーニング時の画像検査でILD が疑われる。 5. 併発する肺疾患に起因する臨床的に重度の呼吸障害(治験責任医師の判定に基づく)が認められる。 6. 副腎皮質ステロイド(プレドニゾン10 mg 超又はそれに相当する抗炎症活性を有するステロイド薬)の長期的な全身投与又は何らかの免疫抑制療法をCycle 1 Day 1 前に受けている。ただし、気管支拡張薬、吸入又は外用ステロイド、もしくはステロイド局所注射を必要とする患者は、本治験に組み入れることができる。 7. HER3抗体及び/又はトポイソメラーゼI阻害剤であるエキサテカン誘導体を構成成分とするADC(例: トラスツズマブ デルクステカン)による治療歴がある。 8. Cycle 1 Day 1前3年以内に、他の活動性悪性腫瘍の既往歴を有する。ただし、以下の悪性腫瘍は例外とする。 a. 適切に治療された非黒色腫皮膚癌 b. 適切に治療された上皮内子宮頸癌 c. 根治的に治療されたその他の上皮内癌 9. 本治験参加によるリスクが高い、又は治験実施計画書の遵守が難しいと治験責任医師が判断するような重症又はコントロール不良の疾患(例: 活動性出血性素因、重篤な活動性感染症)、精神疾患/社会的状況、地理的要因、物質乱用、又はその他の要因が認められる。 |
1. Has HER2-positive gastric cancer as classified by American Society of Clinical Oncology/College of American Pathologists (ASCO-CAP) guidelines and determined prior to enrollment by assessment in a local laboratory that is Clinical Laboratory Improvement Amendments certified (US sites) or accredited based on specific country regulations. 2. Has nasopharyngeal cancer. 3. Has mucosal or uveal melanoma. 4. Has any history of interstitial lung disease (ILD; including pulmonary fibrosis or radiation pneumonitis), has current ILD, or is suspected to have such disease by imaging during screening. 5. Has clinically severe respiratory compromise (based on the investigator's assessment) resulting from intercurrent pulmonary illnesses. 6. Is receiving chronic systemic corticosteroids dosed at >10 mg prednisone or equivalent anti-inflammatory activity or any form of immunosuppressive therapy prior to Cycle 1 Day 1. Subjects who require use of bronchodilators, inhaled or topical steroids, or local steroid injections may be included in the study. 7. Had prior treatment with an anti-HER3 antibody and/or ADC that consists of an exatecan derivative that is a topoisomerase I inhibitor (eg, trastuzumab deruxtecan). 8. Has history of other active malignancy within 3 years prior to Cycle 1 Day 1, except the following: a. Adequately treated nonmelanoma skin cancer b. Adequately treated intraepithelial carcinoma of the cervix c. Any other curatively treated in situ disease. 9. Has any evidence of severe or uncontrolled diseases (eg, active bleeding diatheses, active serious infection) psychiatric illness/social situations, geographical factors, substance abuse, or other factors that, in the investigator's opinion, make it high risk for the subject to participate in the study or that would jeopardize compliance with the protocol. |
