将来的には患者の喘息コントロールや肺機能、さらに日々の自覚症状や服薬に関するトータル管理を行えるモバイル医療機器を開発することを目的とするが、本研究では、1) 貸与したスマートフォンにインストールされた本研究で独自に開発したアプリケーションによる内服や吸入などのし忘れのリマインド機能により治療アドヒアランスが向上するか、また、2) 経時的に変動する喘息患者の症状やコントロールの状態(患者本人が録音する肺音、発声音、咳音、日々のピークフロー値)の記録が可能であるか、を明らかにする。 | |||

① 気管支喘息 ② 年齢:18歳以上 ③ 性別:問わない ④ 重症度は問わないが、増悪(発作)時でないこと ⑤ 本研究への参加にあたり十分な説明を受けた後,十分な理解の上,研究対象者本人の自由意思による文書同意が得られた患者 ⑥ 研究機関長の許可日~2024年1月31日までに当科呼吸器内科外来を受診し、同意取得後も4~6週患の患者状態管理システムの利用が期待される患者。 ⑦ 共同研究機関が開発する患者状態管理システムの利用が可能な患者 |
1. Bronchial asthma 2. Age: 18 years old or older 3. Gender: any gender 4. Any severity of asthma, but not during an exacerbation (attack) 5. Patients who have given a full explanation of their participation in this study, and who have given written consent of their own free will based on a thorough understanding of the subject's condition. 6. Patients who will visit the Respiratory Medicine outpatient clinic of our department from the date of approval by the head of the research institute to January 31, 2024, and are expected to use the patient condition management system for 4 to 6 weeks after obtaining consent. 7. Patients who can use the patient condition management system developed by the collaborating institutions. |

スマートフォンにインストールされた自動的にアルゴリズムによって行われる服薬忘れに対するリマインドアプリケーションの作動の有無(介入あり、なしはランダムに割付する)。介入有り群では、患者毎に設定された吸入回数の入力がない場合に、2回までアプリケーションで服薬リマインドのメッセージ表示(朝の吸入済み、のチェックが入らない場合、"朝の吸入は済みましたか")が自動的に待ち受け画面に表われるプログラムがインストールされたスマートフォンを貸与し、介入なし群では、リマインドメッセージが表示されないスマートフォンを貸与する。いずれも服薬やピークフロー値などの喘息日記を入力する。期間はいずれかのスマートフォンを貸与した日から4~6週間。 | Activation or non-activation of an automatic algorithmic medication reminder application installed on smartphones (with or without intervention to be randomly assigned) | |

役割としては、ソニーグループ株式会社で気管支喘息患者の状態管理システム(スマートフォンアプリケーション)を開発およびシステム機材の提供、収集されたデータの解析、システムの課題抽出を担当する。長崎大学病院呼吸器内科および諫早総合病院で状態管理システムアプリケーションがインストールされたスマートフォンを対象者(気管支喘息患者)へ貸与しバイタルデータ、症状データを収集し個人情報を削除したうえでソニーグループ株式会社に提供する。収集されたデータ、環境データは長崎大学病院呼吸器内科、長崎大学情報データ科学部工学研究科とソニーグループ株式会社の其々で持つ知見を活かし、患者の臨床的呼吸器状態との相関の統計的解析を共同で行う。ソニーグループ株式会社と長崎大学医学部呼吸器内科は、本研究で抽出された患者状態管理システムの課題を分析しシステムのさらなる改善方法を検討する。 | The role of this project is to develop a condition management system (smartphone application) for bronchial asthma patients, provide system equipment, analyze collected data, and identify system issues at Sony Group Inc. Smartphones with the application installed at the Department of Respiratory Medicine, Nagasaki University Hospital and Isahaya General Hospital will be lent to the target patients (bronchial asthma patients) to collect their vital data and symptom data, which will be provided to Sony Group Inc. after removing their personal information. The collected data and environmental data will be statistically analyzed in correlation with the patient's clinical respiratory condition, utilizing the respective expertise of the Department of Respiratory Medicine, Nagasaki University Hospital, the Graduate School of Engineering, Faculty of Information and Data Science, Nagasaki University, and Sony Group Inc. Sony Group Inc. and the Department of Respiratory Medicine, Nagasaki University School of Medicine will analyze the issues of the patient condition management system identified in this study and consider ways to further improve the system. |
