/ 研究対象患者のうち、119例が女性（60.0％）で、104例が60歳以上（52.8％）であった。診断名は、パーキンソン病（69.5％）、多発性硬化症（20.3％）、てんかん（10.2％）で、全体の平均罹病期間は7.4年であった。 Of the patients, 119 were women (60.0%) and 104 were at least 60 years of age (52.8%). The most common diagnosis was Parkinson's disease (69.5%), followed by multiple sclerosis (20.3%) and epilepsy (10.2%), with an overall mean disease duration of 7.4 years.
/ 198例の患者が登録され、197例が最大の解析対象集団に組み入れられた（1例同意取り下げ）。 In total, 198 patients were enrolled and 197 patients were included in the FAS (one patient withdrew their consent).
/ 有害事象の報告なし No reports of adverse events
/ 主要評価項目である、患者の満足度、意思決定の共有、日常生活動作、QOLに関して、患者と医師間の認識のわずかなギャップが確認された。 Slight gaps in perceptions between patients and physicians were identified regarding the primary endpoints of patient satisfaction, shared decision-making, activities of daily living, and quality of life.
/ 本研究は、神経疾患患者とその担当医師との間の認識のギャップとその影響因子の存在を明らかにした。また、研究データに基づき、予測モデルを開発するために、複数の機械学習アルゴリズムを用いた 。本研究から得られた知見は、このような不一致を認識することの重要性を強調するものであり、患者と医師との間のより良い理解を通じて臨床実践を向上させるものである。 This study identified perception gaps between patients with neurological diseases and their physicians and the factors that influence them. Multiple machine learning algorithms were used to develop predictive models based on study data. Insights from this study underscore the importance of recognizing such discrepancies for enhancing clinical practice through a better understanding between patients and their physicians.

1. 対象の疾患と診断された患者：パーキンソン病、多発性硬化症、てんかん（若年性ミオクロニーてんかん、若年性欠伸てんかん、覚醒時大発作てんかん）
2. 順天堂大学医学部附属順天堂医院脳神経内科に通院開始後３か月以上経過し、かつ過去3ヶ月以内に一回以上来院した再診患者
3. 本研究への参加にあたり十分な説明を受けた後、十分な理解の上、研究対象者本人の自由意思による文書同意が得られた方		 1. Diagnosis of target disease: Parkinson's disease, multiple sclerosis, epilepsy (Juvenile myoclonic epilepsy, juvenile absence-like epilepsy, grand mal epilepsy in wakefulness)
2. Patients who have been visiting the Department of Neurology at Juntendo University Hospital for at least 3 months and have visited the hospital at least once within the past 3 months
3. Persons who have received sufficient explanation for participation in this research and have obtained written consent of the subject's own free will after sufficient understanding

主目的である神経疾患患者（特にパーキンソン病、多発性硬化症、てんかん外来患者）と主治医師の間での患者状態の理解度、満足度およびSDMに関する3つのギャップの実態を明らかにするために、患者と医師の双方が対で評価する下記質問票の回答結果を主要評価項目とする。

6. 独自アンケート（患者と医師の両方に対で評価する項目）（Visit 2）		 The primary endpoint will be the results of the following questionnaire, which will be assessed by both patients and physicians in pairs, in order to clarify the actual situation of the three gaps in the level of understanding of the patient's condition, the level of satisfaction, and the SDM between patients with neurological diseases (Particularly outpatients with Parkinson's disease, multiple sclerosis, or epilepsy) and physicians.

副目的として、主要評価項目で明らかになった神経疾患患者（特にパーキンソン病、多発性硬化症、てんかん外来患者）と主治医師の間での患者状態の理解度、満足度およびSDMに関するギャップの影響因子を明らかにするために、下記に示した患者のみが回答する質問票の回答結果、医師の属性情報、及びカルテの必要項目を、副次的評価項目として集計・解析を行う。

7. カルテ情報		 As a secondary objective, in order to clarify the factors affecting the gap in the level of understanding of the patient's condition, the level of satisfaction, and SDM between patients with neurological diseases (Particularly outpatients with Parkinson's disease, multiple sclerosis, or epilepsy) and attending physicians, which was revealed in the primary endpoint, the results of the questionnaire to be answered by only the following patients, physician's attribute information, and necessary items in EMRs will be tabulated and analyzed as secondary endpoints.
