| Methods of Processing Personal Information The principal investigator or principal research scientist will assign a research subject identification code to the patient. The research subject identification code will not contain information that can identify individual research subjects, such as their medical record IDs or names. For the purpose of identification and inquiry of research subjects, a table ("correspondence table") will be created to manage personal information that can identify research subjects from their research subject identification codes. However, the Correspondence Table will not be provided to any person outside the research institution where the Correspondence Table was created. When the results of this research are made public, they will not contain any information that can identify individual research subjects. Transfer of samples/information Case report forms and samples will use the research subject's identification code, and will not include the medical record ID. The case report form should be prepared in electronic media, recorded on non-rewritable electronic media, and stored at the principal investigator's institution. If the case report form is printed and stored in paper form, the date of creation and the signature of the principal investigator should be provided. The revision history (including date and name) shall be accurately recorded and stored on USB. The revision history (including date and name) should be accurately recorded and saved on USB. The collaborating institutions do not register research subjects. Therefore, no registration forms or case report forms will be exchanged. |
