本治験はNSCLC患者と対象とした経口EGFR/HER2 阻害薬TAK-788の安全性、薬物動態及び抗腫瘍活性、並びにEGFR又はHER2変異を有する固形がん患者を対象としたTAK-788の抗腫瘍活性を検討する第1/2相試験である。本治験は、用量漸増パート(パート1)、拡大パート(パート2)及び延長パート(パート3)の3 つのパートから構成される。なお、日本では延長パートのみ実施する。 用量漸増パート(パート1)の目的は、TAK-788 を経口投与したときの安全性プロファイル(最大耐量 [MTD]、用量制限毒性 [DLT] 及び第2 相推奨用量 [RP2D]を含む)、薬物動態プロファイルを明らかにすることである。拡大パートの主目的は、組織学的及び分子生物学的に定義された7つのコホートにおけるTAK-788 のRP2D(本治験の用量漸増パートで決定)を投与したときの抗腫瘍活性を検討することである。 延長パートの7つのコホートは以下の通りである: 1. EGFR のエクソン20 挿入変異を有し、EGFR TKI による治療を受けていない、又はEGFR TKI による客観的奏効を達成したことがない、測定可能又は活動性のCNS転移が認められないNSCLC 患者 2. HER2 のエクソン20 挿入変異又は点変異を有し、測定可能又は活動性のCNS 転移が認められないNSCLC 患者 3. EGFR のエクソン20 挿入変異又はHER2 のエクソン20 挿入変異若しくは点変異を有し、測定可能又は活動性のCNS 転移が認められるNSCLC 患者 4. TAK-788 に感受性を示すその他の標的〔例:EGFR のエクソン19 欠失若しくはエクソン21 置換(T790M変異の有無は問わない)、又はその他の低頻度活性型変異〕を有する、CNS転移が認められないNSCLC 患者 5. EGFR のエクソン20 挿入変異を有し、過去にEGFR TKI により客観的奏効を達成し、その後病勢進行した、CNS転移が認められないNSCLC 患者 6. EGFR のエクソン20 挿入変異を有し、過去に局所進行病変又は転移性病変に対して全身性抗悪性腫瘍治療を行っていない、CNS転移が認められないNSCLC 患者 7. TAK-788 に感受性を示すEGFR/HER2 変異を有し、CNS転移が認められないNSCLC 以外の固形がん患者 延長パートは、EGFR のエクソン20 挿入変異を有し、治療歴がある局所進行性又は転移性NSCLC 患者にTAK-788を投与したときの有効性を検討する。本治験には324例の患者が登録された。 |

本治験はNSCLC患者と対象とした経口EGFR/HER2 阻害薬TAK-788の安全性、薬物動態及び抗腫瘍活性、並びにEGFR又はHER2変異を有する固形がん患者を対象としたTAK-788の抗腫瘍活性を検討する第1/2相試験である。本治験は、用量漸増パート(パート1)、拡大パート(パート2)及び延長パート(パート3)の3 つのパートから構成される。なお、日本では延長パートのみ実施する。 用量漸増パート(パート1)の目的は、TAK-788 を経口投与したときの安全性プロファイル(最大耐量 [MTD]、用量制限毒性 [DLT] 及び第2 相推奨用量 [RP2D]を含む)、薬物動態プロファイルを明らかにすることである。拡大パートの主目的は、組織学的及び分子生物学的に定義された7つのコホートにおけるTAK-788 のRP2D(本治験の用量漸増パートで決定)を投与したときの抗腫瘍活性を検討することである。 延長パートの7つのコホートは以下の通りである: 1. The trial will be conducted in three parts: a dose escalation (Part 1), expansion phase (Part 2), followed by an extension phase (Part 3). The objectives of the dose escalation phase (Part 1) a is to determine the safety profile of orally administered TAK-788, including the Maximum Tolerated Dose (MTD), Dose Limiting Toxicities (DLTs), Recommended Phase 2 Dose (RP2D), pharmacokinetic profile. The primary goal of the expansion component of the trial is to evaluate the anti-tumor activity of TAK-788 in seven histologically and molecularly defined cohorts at the RP2D (determined based on dose escalation phase of the trial). The seven expansion cohorts will be: 1. NSCLC participants with EGFR exon 20 activating insertions, who have either not received or not shown an objective response to an EGFR TKI, and who have no active, measurable CNS metastases; 2. NSCLC participants with HER2 exon 20 activating insertions or point mutations and no active, measurable CNS metastases; 3. NSCLC participants with EGFR exon 20 activating insertions or HER2 exon 20 activating insertions or point mutations and active, measurable CNS metastases; 4. NSCLC participants with other targets against which TAK-788 is active (examples include EGFR exon 19 deletions or exon 21 substitutions [with or without T790M mutations] and other uncommon EGFR activating mutations), without active CNS metastases; 5. NSCLC participants with EGFR exon 20 activating insertions, who have previously shown an objective response to an EGFR TKI and subsequently progressed, without active CNS metastases; 6. NSCLC participants with EGFR exon 20 activating insertions, who have not received prior systemic anticancer treatment for locally advanced or metastatic disease, without active CNS metastases; and 7. Participants with solid tumors other than NSCLC with EGFR/HER2 mutations against which TAK-788 is active, without active CNS metastases. The extension phase will evaluate efficacy of TAK-788 in participants with locally advanced or metastatic NSCLC whose tumors harbor EGFR exon 20 insertion mutations and who have been previously treated. The study enrolled 324 participants. | ||
