本治験の被検薬はTAK-019であり、新型コロナウイルス(SARS-CoV-2)による感染症の予防を予定とされる適応症としている。本治験では日本人健康成人男女を対象に、TAK-019を21日間隔で2回筋肉内接種したときの安全性及び免疫原性を評価する。

/ 組み入れた200例はすべて日本人であった。このうち、114例(57.0%)が男性、86例(43.0%)が女性であり、TAK-019群の150例中85例(56.7%)及びプラセボ群の50例中29例(58.0%)が男性であった。平均年齢は52.2+-15.93歳であった。TAK-019群及びプラセボ群の平均年齢はそれぞれ52.6+-15.80歳及び50.8+-16.42歳であった。年齢層については、200例中140例(70.0%)は20歳以上65歳未満、200例中60例(30.0%)は65歳以上であった。TAK-019群の150例中100例(66.7%)及びプラセボ群の50例中40例(80.0%)は20歳以上65歳未満、TAK-019群の150例中50例(33.3%)及びプラセボ群の50例中10例(20.0%)は65歳以上であった。
全体として、人口統計学的及び他の基準値の特性は2群間で同様であった。
All 200 subjects enrolled were Japanese. Of them, 114 (57.0%) subjects were male and 86 (43.0%) subjects were female; 85 (56.7%) of 150 subjects in the TAK-019 arm and 29 (58.0%) of 50 subjects in the placebo arm were male. Mean age was 52.2+-15.93 years; mean ages of the TAK-019 and placebo arms were 52.6+-15.80 and 50.8+-16.42 years, respectively. For age group, 140 (70.0%) of 200 subjects were 20 to <65 years of age, and 60 (30.0%) of 200 subjects were >=65 years; 100 (66.7%) of 150 subjects in the TAK-019 arm and 40 (80.0%) of 50 subjects in the placebo arm were 20 to <65 years, and 50 (33.3%) of 150 subjects in the TAK-019 arm and 10 (20.0%) of 50 subjects in the placebo arm were >=65 years.
Overall, subject demographic and other baseline characteristics were similar between the 2 arms.
/ 合計326例をスクリーニングし、200例を無作為化した。このうち、150例をTAK-019群に、50例をプラセボ群に組み入れた。TAK-019群では、150例(100%)がTAK-019の1回目及び2回目の接種を受けた。TAK-019群の19例(12.7%)が主に「その他」のために早期に治験を中止した。「その他」の理由は、公的に入手可能なSARS-CoV-2ワクチンの接種を被験者が希望したためであった。プラセボ群では、50例(100%)がプラセボの1回目の接種を受け、49例(98.0%)がプラセボの2回目の接種を受けた。このうち、1例(2.0%)は1回目接種後の「有害事象(耳鳴)」のため接種を中止した。プラセボ群の46例(92.0%)が「その他」のために早期に治験を中止した。「その他」の理由は、公的に入手可能なSARS-CoV-2ワクチンの接種を被験者が希望したためであった。 A total of 326 subjects were screened and 200 subjects were randomized. Of them, 150 subjects were enrolled in the TAK-019 arm and 50 subjects were enrolled in the placebo arm. In the TAK-019 arm, 150 (100%) subjects received the first and second doses of TAK-019. In the TAK-019 arm, 19 (12.7%) subjects discontinued early from the study mainly due to "Other" category; the reason in the category of "Other" was subject's request to take publicly available SARS-CoV-2 vaccine. In the placebo arm, 50 (100%) subjects received the first dose of placebo and 49 (98.0%) subjects received the second dose of placebo. Of them, 1 (2.0%) subject discontinued vaccination due to "Adverse event (tinnitus)" after the first vaccination. In the placebo arm, 46 (92.0%) subjects discontinued early from the study due to "Other" category; the reason in the category of "Other" was subject's request to take publicly available SARS-CoV-2 vaccine.
/ 注射部位の特定有害事象:
注射部位の特定有害事象は、いずれかの接種から7日後までに、TAK-019群の150例中110例(73.3%)及びプラセボ群の50例中4例(8.0%)で認められた。両群とも、最も高い重症度の注射部位の特定有害事象の大多数はグレード2以下であった。最も多く報告された(上位3つ)注射部位の特定有害事象は、TAK-019群では、圧痛(100例[66.7%])、その次が注射部位疼痛(88例〔58.7%〕)及び腫脹(27例[18.0%])であった。プラセボ群で報告された注射部位の特定有害事象は、注射部位疼痛及び圧痛(それぞれ3例[6.0%])であった。
1回目の接種から7日後まででは、注射部位の特定有害事象は、TAK-019群の150例中76例(50.7%)及びプラセボ群の50例中3例(6.0%)で認められた。最も多く報告された(上位3つ)注射部位の特定有害事象は、TAK-019群では、圧痛(65例[43.3%])、その次が注射部位疼痛(44例[29.3%])及び硬結(5例[3.3%])であった。プラセボ群で報告された注射部位の特定有害事象は、注射部位疼痛及び圧痛(それぞれ2例[4.0%])であった。2回目の接種から7日後まででは、注射部位の特定有害事象は、TAK-019群の150例中103例(68.7%)及びプラセボ群の49例中2例(4.1%)で認められた。最も多く報告された(上位3つ)注射部位の特定有害事象は、TAK-019群では、圧痛(94例[62.7%])、その次が注射部位疼痛(75例[50.0%])及び腫脹(26例[17.3%])であった。プラセボ群で報告された注射部位の特定有害事象は、圧痛(2例[4.1%])及び注射部位疼痛(1例[2.0%])であった。

全身性の特定有害事象:
全身性の特定有害事象は、いずれかの接種から7日後までに、TAK-019群の150例中86例(57.3%)及びプラセボ群の50例中11例(22.0%)で認められた。両群とも、最も高い重症度の全身性の特定有害事象の大多数はグレード2以下であった。最も多く報告された(上位3つ)全身性の特定有害事象は、TAK-019群では、筋肉痛(56例[37.3%])、その次が倦怠感(50例[33.3%])及び頭痛(41例[27.3%])であった。プラセボ群で最も多く報告された(上位3つ)全身性の特定有害事象は、疲労(7例[14.0%])、その次が倦怠感(5例[10.0%])及び筋肉痛(4例[8.0%])であった。
1回目の接種から7日後まででは、全身性の特定有害事象は、TAK-019群の150例中44例(29.3%)及びプラセボ群の50例中6例(12.0%)で認められた。最も多く報告された(上位3つ)全身性の特定有害事象は、TAK-019群では、筋肉痛(26例[17.3%])、その次が頭痛(16例[10.7%])及び倦怠感(15例[10.0%])であった。プラセボ群で最も多く報告された(上位3つ)全身性の特定有害事象は、疲労(3例[6.0%])、その次が倦怠感及び筋肉痛(それぞれ2例[4.0%])であった。2回目の接種から7日後まででは、全身性の特定有害事象は、TAK-019群の150例中75例(50.0%)及びプラセボ群の49例中6例(12.2%)で認められた。最も多く報告された(上位3つ)全身性の特定有害事象は、TAK-019群では、筋肉痛(49例[32.7%])、その次が倦怠感(44例[29.3%])及び頭痛(32例[21.3%])であった。プラセボ群で最も多く報告された(上位3つ)全身性の特定有害事象は、疲労(4例[8.2%])、その次が倦怠感(3例[6.1%])及び筋肉痛(2例[4.1%])であった。

非特定有害事象:
1回目の接種からDay 50までに、非特定有害事象は、TAK-019群の150例中54例(36.0%)及びプラセボ群の50例中9例(18.0%)で認められた。このうち、治験薬との関連ありの非特定有害事象は、TAK-019群の39例(26.0%)及びプラセボ群の3例(6.0%)で認められた。TAK-019群及びプラセボ群で、被験者により報告された最も高い重症度の非特定有害事象はいずれもグレード2以下であった。中止については、治験薬の接種中止に至った非特定有害事象は、プラセボ群の1例(2.0%)で認められ、治験中止に至った非特定有害事象は両群で認められなかった。
1回目の接種からDay 50までに、最も多く報告された(上位3つ)非特定有害事象のSOCは、TAK-019群では、「一般・全身障害および投与部位の状態」(35例[23.3%])、その次が「胃腸障害」(8例[5.3%])及び「感染症および寄生虫症」(5例[3.3%])であった。プラセボ群で、最も多く報告された非特定有害事象のSOCは、「一般・全身障害および投与部位の状態」(3例[6.0%])であった。最も多く報告された(上位3つ)非特定有害事象は、TAK-019群では、注射部位そう痒感(26例[17.3%])、その次が下痢及び悪寒(5例[3.3%])であった。
1回目の接種から21日後まででは、最も多く報告された(上位3つ)非特定有害事象は、TAK-019群では、注射部位そう痒感(5例[3.3%])、その次が下痢(3例[2.0%])並びに注射部位腫脹及び上咽頭炎(それぞれ2例[1.3%])であった。2回目の接種から28日後まででは、最も多く報告された(上位3つ)非特定有害事象は、注射部位そう痒感(24例[16.0%])、その次が悪寒(5例[3.3%])並びに注射部位熱感及び上咽頭炎(3例[2.0%])であった。

死亡、その他の重篤な有害事象、及び他の重要な有害事象:
治験期間を通して、死亡、治験中止に至った有害事象、又は注目すべき有害事象は認められなかった。
重篤な有害事象は、Day 50以降治験終了までに、プラセボ群の1例で1件(COVID-19、入院が必要)報告された。本事象は消失し、治験責任医師は治験ワクチンとの因果関係はないと判断した。
治験薬の接種中止に至った有害事象は、プラセボ群の1例で、1回目の接種後に1件(耳鳴)報告され、当該被験者は2回目の接種を受けなかった。
診療を要した有害事象は、いずれかの接種後、1回目の接種からDay 50までに、それぞれTAK-019群の4例(2.7%)及びプラセボ群の2例(4.0%)で認められた。2例以上で発現した診療を要した有害事象は、TAK-019群では齲歯(2例〔1.3%〕)で、プラセボ群では認められなかった。治験期間を通して、診療を要した有害事象はそれぞれTAK-019群の16例(10.7%)及びプラセボ群の3例(6.0%)で認められた。2例以上で発現した診療を要した有害事象は、TAK-019群では齲歯及びCOVID-19(それぞれ3例〔2.0%〕)、咽頭炎及び糖尿病(それぞれ2例〔1.3%〕)で、プラセボ群では認められなかった。
1回目の接種後、Day 50までにSARS-CoV-2に感染した被験者は報告されなかった。Day 50以降、TAK-019群の4例(2.7%)、プラセボ群の1例(2.0%)でCOVID-19(SARS-CoV-2感染)が認められた。このうち、プラセボ群の1例の事象が重篤と判断された。COVID-19が認められたTAK-019群の4例については、2例が中等度、2例が軽度であった。両群の被験者で発現したCOVID-19はいずれも、治験ワクチンとの因果関係はないと判断され、追跡調査中に消失した。
Solicited Local Adverse Events:
For 7 days following any vaccination, 110 (73.3%) subjects had solicited local AEs in 150 subjects in the TAK-019 arm and 4 (8.0%) subjects had solicited local AEs in 50 subjects in the placebo arm. The majority of solicited local AEs with the maximum severity were Grade =<2 in both arms. The most frequently reported (up to the 3rd most) solicited local AEs were tenderness (100 [66.7%] subjects), followed by injection site pain (88 [58.7%] subjects) and swelling (27 [18.0%] subjects) in the TAK-019 arm. In the placebo arm, solicited local AEs reported were: injection site pain and tenderness (3 [6.0%] subjects, each).
For 7 days following the first vaccination, 76 (50.7%) subjects had solicited local AEs in 150 subjects in the TAK- 019 arm and 3 (6.0%) subjects had solicited local AEs in 50 subjects in the placebo arm. The most frequently reported (up to the 3rd most) solicited local AEs were tenderness (65 [43.3%] subjects), followed by injection site pain (44 [29.3%] subjects) and induration (5 [3.3%] subjects) in the TAK-019 arm. In the placebo arm, solicited local AEs reported were: injection site pain and tenderness (2 [4.0%] subjects, each). For 7 days following the second vaccination, 103 (68.7%) subjects had solicited local AEs in 150 subjects in the TAK-019 arm and 2 (4.1%) subjects had solicited local AEs in 49 subjects in the placebo arm. The most frequently reported (up to the 3rd most) solicited local AEs were tenderness (94 [62.7%] subjects), followed by injection site pain (75 [50.0%] subjects) and swelling (26 [17.3%] subjects) in the TAK-019 arm. In the placebo arm, solicited local AEs reported were: tenderness (2 [4.1%] subjects) and injection site pain (1 [2.0%] subject).

Solicited Systemic Adverse Events:
For 7 days following any vaccination, 86 (57.3%) subjects had solicited systemic AEs in 150 subjects in the TAK-019 arm and 11 (22.0%) subjects had solicited systemic AEs in 50 subjects in the placebo arm. The majority of solicited systemic AEs with the maximum severity were Grade =<2 in both arms. The most frequently reported (up to the 3rd most) solicited systemic AEs were myalgia (56 [37.3%] subjects), followed by malaise (50 [33.3%] subjects) and headache (41 [27.3%] subjects) in the TAK-019 arm. In the placebo arm, the most frequently reported (up to the 3rd most) solicited systemic AEs were fatigue (7 [14.0%] subjects), followed by malaise (5 [10.0%] subjects) and myalgia (4 [8.0%] subjects).
For 7 days following the first vaccination, 44 (29.3%) subjects had solicited systemic AEs in 150 subjects in the TAK-019 arm and 6 (12.0%) subjects had solicited systemic AEs in 50 subjects in the placebo arm. The most frequently reported (up to the 3rd most) solicited systemic AEs were myalgia (26 [17.3%] subjects), followed by headache (16 [10.7%] subjects) and malaise (15 [10.0%] subjects) in the TAK-019 arm. In the placebo arm, the most frequently reported (up to the 3rd most) solicited systemic AEs were fatigue (3 [6.0%] subjects), followed by malaise and myalgia (2 [4.0%] subjects, each). For 7 days following the second vaccination, 75 (50.0%) subjects had solicited systemic AEs in 150 subjects in the TAK-019 arm and 6 (12.2%) subjects had solicited systemic AEs in 49 subjects in the placebo arm. The most frequently reported (up to the 3rd most) solicited systemic AEs were myalgia (49 [32.7%] subjects), followed by malaise (44 [29.3%] subjects) and headache (32 [21.3%] subjects) in the TAK-019 arm. In the placebo arm, the most frequently reported (up to the 3rd most) solicited systemic AEs were fatigue (4 [8.2%] subjects), followed by malaise (3 [6.1%] subjects) and myalgia (2 [4.1%] subjects).

Unsolicited Adverse Events:
After any vaccination until Day 50 from the first vaccination, 54 (36.0%) subjects had unsolicited AEs in 150 subjects in the TAK-019 arm and 9 (18.0%) subjects had unsolicited AEs in 50 subjects in the placebo arm. Of these, 39 (26.0%) subjects had treatment-related unsolicited AEs in the TAK-019 arm and 3 (6.0%) subjects had treatment-related unsolicited AEs in the placebo arm. All unsolicited AEs with the maximum severity reported by subjects were Grade =<2 in the TAK-019 and placebo arms. For discontinuation/withdrawal, 1 (2.0%) subject had unsolicited AEs leading to discontinuation of vaccination in the placebo arm, and no subjects had unsolicited AEs leading to withdrawal from the trial in both arms.
After any vaccination until Day 50 from the first vaccination, the most frequently reported (up to the 3rd most) unsolicited AEs were in the SOC of "General disorders and administration site conditions" (35 [23.3%] subjects), followed by "Gastrointestinal disorders" (8 [5.3%] subjects) and "Infections and infestations" (5 [3.3%] subjects) in the TAK-019 arm. In the placebo arm, "General disorders and administration site conditions" (3 [6.0%] subjects) was the most frequently reported unsolicited AEs in the SOC. The most frequently reported (up to the 3rd most) unsolicited AEs were injection site pruritus (26 [17.3%] subjects), followed by diarrhoea and chills (5 [3.3%] subjects, each) in the TAK-019 arm.
For 21 days following the first vaccination, the most frequently reported (up to the 3rd most) unsolicited AEs were injection site pruritus (5 [3.3%] subjects), followed by diarrhoea (3 [2.0%] subjects), and injection site swelling and nasopharyngitis (2 [1.3%] subjects, each) in the TAK-019 arm. For 28 days following the second vaccination, the most frequently reported (up to the 3rd most) unsolicited AEs were injection site pruritus (24 [16.0%] subjects), followed by chills (5 [3.3%] subjects), and injection site warmth and nasopharyngitis (3 [2.0%] subjects, each).

Deaths, Other Serious Adverse Events, and Other Significant Adverse Events:
No deaths, AEs leading to subject's withdrawal from study, or AESIs occurred throughout the trial. One SAE (COVID-19 required for hospitalization) was reported in a subject of the placebo arm after Day 50 until the end of the trial. The event was resolved and the Investigator judged to have no causality with the study vaccine.
One AE leading to discontinuation of vaccination (tinnitus) was reported in 1 subject in the placebo arm after the first vaccination, and the subject did not receive the second vaccination.
After any vaccination until Day 50 from the first vaccination, 4 (2.7%) subjects and 2 (4.0%) subjects had MAAEs in the TAK-019 arm and in the placebo arm, respectively. MAAEs occurring in >=2 subjects were dental caries (2 [1.3%] subjects) in the TAK-019 arm and no events in the placebo arm. Throughout the trial, 16 (10.7%) subjects and 3 (6.0%) subjects had MAAEs in the TAK-019 arm and in the placebo arm, respectively. MAAEs occurring in >=2 subjects were dental caries and COVID-19 (3 [2.0%] subjects, each), pharyngitis and diabetes mellitus (2 [1.3%] subjects, each) in the TAK-019 arm and no events in the placebo arm.
No SARS-CoV-2 infected subjects were reported until Day 50 from the first vaccination. After Day 50, 4 (2.7%) subjects in the TAK-019 arm and 1 (2.0%) subject in the placebo arm had COVID-19 (SARS-CoV-2 infection). Of them, 1 subject in the placebo arm had the event considered serious. For 4 subjects in the TAK-019 arm who had COVID-19, the severities of these events were moderate in 2 subjects and mild in 2 subjects. All COVID-19 events that occurred in subjects in both arms were judged to have no causality with the study vaccine and resolved during the follow-up.
/ - 免疫原性の主要評価項目
Day 36におけるSARS-CoV-2 rSタンパクに対する血清IgG抗体価のGMT、GMFR、SCR、及びSRR:
SARS-CoV-2 rSタンパクに対する血清IgG抗体価は、本治験での「治験実施計画書に適合した対象集団」(TAK-019群150例、プラセボ群49例)において測定した。TAK-019群の血清IgG抗体価のGMTは、ベースライン時に1 mLあたりの酵素結合性免疫吸着検定法(ELISA)単位で119.9 EU/mL(95%信頼区間:110.7、130.0)であった。Day 36におけるTAK-019群のGMTは31036.8 EU/mL(95%信頼区間:26837.1、35893.7)であった。プラセボ群では、血清IgG抗体価のGMTは、ベースライン時に128.4 EU/mL(95%信頼区間:107.0、154.0)、Day 36に132.3 EU/mL(95%信頼区間:109.6、159.5)であった。ベースラインからDay 36までのGMFRは、TAK-019群では258.8(95%信頼区間:218.8、306.0)、プラセボ群では1.0(95%信頼区間:1.0、1.1)であった。
Day 36の血清IgG抗体価のSCRは、TAK-019群及びプラセボ群でそれぞれ、100%(95%信頼区間:97.6、100.0)及び0%(95%信頼区間:0.0、7.3)であった。Day 36の血清IgG抗体価のSRRは、TAK-019群及びプラセボ群でそれぞれ、100%(95%信頼区間:97.6、100.0)及び8.2%(95%信頼区間:2.3、19.6)であった。

- 免疫原性の副次評価項目
Day 22、Day 50、Day 202及びDay 387におけるSARS-CoV-2 rSタンパクに対する血清IgG抗体価のGMT、GMFR、SCR、及びSRR:
Day 22、Day 50、Day 202及びDay 387における血清IgG抗体価のGMTは、TAK-019群では、それぞれ1390.0 EU/mL(95%信頼区間:1140.8、1693.7)、23910.8 EU/mL(95%信頼区間:20737.0、27570.3)、4390.1 EU/mL(95%信頼区間:3600.9、5352.3)、及び3461.2 EU/mL(95%信頼区間:2601.5、4605.1)であった。プラセボ群では、Day 22、Day 50、Day 202及びDay 387におけるGMTは、それぞれ144.9 EU/mL(95%信頼区間:115.2、182.4)、134.7 EU/mL(95%信頼区間:109.3、166.2)、851.0 EU/mL(95%信頼区間:26.0、27800.8)、及び236.7 EU/mL(95%信頼区間:15.2、3674.6)であった。ベースラインからDay 22、Day 50、Day 202、及びDay 387までのGMFRは、TAK-019群ではそれぞれ11.6(95%信頼区間:9.4、14.2)、199.1(95%信頼区間:169.2、234.3)、36.4(95%信頼区間:29.8、44.6)、及び27.9(95%信頼区間:20.7、37.6)であった。プラセボ群では、ベースラインからDay 22、Day 50、Day 202、及びDay 387までのGMFRは、それぞれ1.1(95%信頼区間:1.0、1.2)、1.0(95%信頼区間:1.0、1.1)、8.5(95%信頼区間:0.3、278.0)、及び2.4(95%信頼区間:0.2、36.7)であった。

Day 22、Day 36、Day 50、Day 202及びDay 387における野生型ウイルスに対する血清中和抗体価のGMT、GMFR、SCR、及びSRR;

- Primary Immunogenicity Endpoints:
GMT, GMFR, SCR, and SRR of Serum IgG Antibody Levels to SARS-CoV-2 rS Protein on Day 36:
Serum IgG antibody levels to SARS-CoV-2 rS protein were measured for the PPS (150 subjects in the TAK-019 arm and 49 subjects in the placebo arm) in the study. GMT of serum IgG antibody in the TAK-019 arm was 119.9 enzyme-linked immunosorbent assay (ELISA) units per mL (EU/mL) (95% CI: 110.7, 130.0) at the baseline. On Day 36, GMT in the TAK-019 arm was 31036.8 EU/mL (95% CI: 26837.1, 35893.7). In the placebo arm, GMTs of serum IgG antibody were 128.4 EU/mL (95% CI: 107.0, 154.0) at the baseline, and 132.3 EU/mL (95% CI: 109.6, 159.5) on Day 36. GMFRs from baseline to Day 36 were 258.8 (95% CI: 218.8, 306.0) in the TAK-019 arm, and 1.0 (95% CI: 1.0, 1.1) in the placebo arm.
SCRs of serum IgG antibody on Day 36 in the TAK-019 and placebo arms were 100% (95% CI: 97.6, 100.0) and 0% (95% CI: 0.0, 7.3), respectively. SRRs of serum IgG antibody on Day 36 in the TAK-019 and placebo arms were 100% (95% CI: 97.6, 100.0) and 8.2% (95% CI: 2.3, 19.6).

- Secondary Immunogenicity Endpoints
GMT, GMFR, SCR, and SRR of Serum IgG Antibody Levels to SARS-CoV-2 rS Protein on Day 22, Day 50, Day 202, and Day 387:
GMTs of serum IgG antibody on Day 22, Day 50, Day 202, and Day 387 were 1390.0 EU/mL (95% CI: 1140.8, 1693.7), 23910.8 EU/mL (95% CI: 20737.0, 27570.3), 4390.1 EU/mL (95% CI: 3600.9, 5352.3), and 3461.2 EU/mL (95% CI: 2601.5, 4605.1) in the TAK-019 arm, respectively. In the placebo arm, GMTs on Day22, Day 50, Day 202, and Day 387 were 144.9 EU/mL (95% CI: 115.2, 182.4), 134.7 EU/mL (95% CI: 109.3, 166.2), 851.0 EU/mL (95% CI: 26.0, 27800.8), and 236.7 EU/mL (95% CI: 15.2, 3674.6). GMFRs from baseline to Day 22, to Day 50, to Day 202, and to Day 387 were 11.6 (95% CI: 9.4, 14.2), 199.1 (95% CI: 169.2, 234.3), 36.4 (95% CI: 29.8, 44.6), and 27.9 (95% CI: 20.7, 37.6) in the TAK-019 arm, respectively. In the placebo arm, GMFRs from baseline to Day 22, to Day 50, to Day 202, and to Day 387 were 1.1 (95% CI: 1.0, 1.2), 1.0 (95% CI: 1.0, 1.1), 8.5 (95% CI: 0.3, 278.0), and 2.4 (95% CI: 0.2, 36.7).
SCRs of serum IgG antibody on Day 22, Day 50, Day 202, and Day 387 were 82.7% (95% CI: 75.6, 88.4), 100% (95% CI: 97.6, 100.0), 97.9% (95% CI: 94.1, 99.6), and 96.9% (95% CI: 92.1, 99.1) in the TAK-019 arm, respectively. In the placebo arm, SCRs on Day 22, Day 50, Day 202, and Day 387 were 6.1% (95% CI: 1.3, 16.9), 2.0% (95% CI: 0.1, 10.9), 33.3% (95% CI: 4.3, 77.7), and 25.0% (95% CI: 0.6, 80.6). SRRs of serum IgG antibody on Day 22, Day 50, Day 202, and Day 387 were 84.0% (95% CI: 77.1, 89.5), 100% (95% CI: 97.6, 100.0), 97.9% (95% CI: 94.1, 99.6), and 96.9% (95% CI: 92.1, 99.1) in the TAK-019 arm, respectively. In the placebo arm, SRRs on Day 22, Day 50, Day 202, and Day 387 were 10.2% (95% CI: 3.4, 22.2), 12.2% (95% CI: 4.6, 24.8), 33.3% (95% CI: 4.3, 77.7), and 25.0% (95% CI: 0.6, 80.6).

GMT, GMFR, SCR, and SRR of Serum Neutralizing Antibody Titers to Wild-type Virus on Day 22, Day 36, Day 50, Day 202, and Day 387:
Serum neutralizing antibody titers to wild-type virus (MN50) were evaluated for the PPS (150 subjects in the TAK-019 arm and 49 subjects in the placebo arm) in the study. Titer values measured as below the lower limit of quantification (LLOQ) (= 20) were imputed to a value that was half of the LLOQ. GMT of serum neutralizing antibody at the baseline in the TAK-019 arm was 10.0 (95% CI: 10.0, 10.1). GMTs of serum neutralizing antibody on Day 22, Day 36, Day50, Day 202, and Day 387 were 50.2 (95% CI: 41.2, 61.0), 884.4 (95% CI: 749.0, 1044.4), 509.5 (95% CI: 422.5, 614.6), 65.5 (95% CI: 53.4, 80.4), and 65.7 (95% CI: 49.0, 88.1) in the TAK-019 arm, respectively. In the placebo arm, GMT of serum neutralizing antibody at the baseline was 10.1 (95% CI: 9.9, 10.4). In the placebo arm, SCRs on Day 22, Day36, Day 50, Day 202, and Day 387 were 0% (95% CI: 0.0, 7.3), 0% (95% CI: 0.0, 7.3), 0% (95% CI: 0.0, 7.3), 33.3% (95% CI: 4.3, 77.7), and 25.0% (95% CI: 0.6, 80.6). SRRs of serum neutralizing antibody on Day 22, Day 36, Day 50, Day 202, and Day 387 were 100% (95% CI: 97.6, 100.0), 100% (95% CI: 97.6, 100.0), 100% (95% CI: 97.6, 100.0), 100% (95% CI: 97.5, 100.0), and 100% (95% CI: 97.1, 100.0) in the TAK-019 arm, respectively. In the placebo arm, SRRs on Day 22, Day 36, Day 50, Day 202, and Day 387 were 100% (95% CI: 92.7, 100.0), 100% (95% CI: 92.7, 100.0), 100% (95% CI: 92.7, 100.0), 100% (95% CI: 54.1, 100.0), and 100% (95% CI: 39.8, 100.0). SRR is defined as proportion of subjects with equal or more than the 95th percentile in titer at baseline for all subjects (across the treatment arms). As the 95th percentile in serum neutralizing antibody titers to wild-type virus at baseline was 10 (= half of the LLOQ), all subjects in both arms met the definition of the seroresponse of serum neutralizing antibody in all visits. Therefore, it was hard to interpret the SRR of serum neutralizing antibody in this study.

2 試験等の目的及び内容並びにこれに用いる医薬品等の概要

(1)試験等の目的及び内容

本治験の被検薬はTAK-019であり、新型コロナウイルス(SARS-CoV-2)による感染症の予防を予定とされる適応症としている。本治験では日本人健康成人男女を対象に、TAK-019を21日間隔で2回筋肉内接種したときの安全性及び免疫原性を評価する。

2. 治験薬接種前30日以内にCOVID-19患者と濃厚接触した者
3. 過去にSARS-CoV-2感染陽性と判定された者、又は、治験薬接種前の検査においてSARS-CoV-2感染陽性と判定された者
4. COVID-19の予防のため、現在、その他の治験薬の投与を受けている者
5. 治験参加前30日以内に海外渡航歴のある者
6. 治験薬接種前3日以内に、臨床的に重大な活動性感染症(治験責任医師又は治験分担医師による評価)を有する者又は口腔内体温が38度以上の者
7. 治験薬に含まれる成分に対して過敏症又はアレルギーを有する者
8. 治験責任医師又は治験分担医師より、治験の結果を妨げる、又は治験への参加により被験者に追加的なリスクをもたらす可能性のある病歴又は何らかの疾患を有すると判断された者
9. 自己免疫疾患又は神経炎症性疾患の既往歴を含む、免疫機能の障害又は変化の既往がある者、若しくは疑われる者

1. Participants who received any other SARS-CoV-2 or other experimental novel coronavirus vaccine prior to the trial.
2. Participants who have close contact of anyone known to have COVID-19 within 30 days prior to the trial vaccination.
3. Participants who were tested positive for SARS-CoV-2 prior to the trial or before the trial vaccination.
4. Participants who are on current treatment with other investigational agents for prophylaxis of COVID-19.
5. Participants who have traveled outside of Japan in the 30 days prior to the trial participation.
6. Participants with a clinically significant active infection (as assessed by the Investigator) or oral temperature >= 38 degree Celsius within 3 days of the intended date of vaccination.
7. Participants with known hypersensitivity or allergy to any of the investigational vaccine components.
8. Participants with history or any illness that, in the opinion of the Investigator, might interfere with the results of the trial or pose additional risk to the participants due to participation in the trial.
9. Participants with known or suspected impairment/alteration of immune function, including history of any autoimmune disease or neuro-inflammatory disease.
10. Abnormalities of splenic or thymic function.
11. Participants with a known bleeding diathesis, or any condition that may be associated with a prolonged bleeding time.
12. Participants with any serious chronic or progressive disease (eg, neoplasm, insulin dependent diabetes, cardiac, renal or hepatic disease).
13. Participants with body mass index (BMI) greater than or equal to 30 kg/m^2 (BMI= weight in kg/ height in meters^2).
14. Participants participating in any clinical trial with another investigational product within 30 days prior to the trial vaccination or intend to participate in another clinical trial at any time during the conduct of this trial.
15. Participants who received or plan to receive any other licensed vaccines within 14 days (for inactivated vaccines) or 28 days (for live vaccines) prior to trial dose administration.
