2 試験等の目的及び内容並びにこれに用いる医薬品等の概要

(1)試験等の目的及び内容

本治験の目的は、組換えヒトヒアルロニダーゼPH20(rHuPH20)の配合剤(SC-CF)として皮下投与するamivantamabの抗腫瘍活性及び安全性を評価し(コホート4以外の全コホート)、amivantamab SC-CFの安全性を明らかにする(コホート4)ことである。

・ 組織学的又は細胞学的に確認され,外科的切除又は化学放射線療法を含む根治療法の適応とならない局所進行又は転移性NSCLCを有している患者。その他,コホート別の疾患要件には以下が含まれる:
コホート1,3,3b,5,6及び7:EGFRエクソン19欠失変異又はエクソン21のL858R変異を有する。
コホート2:EGFRエクソン20挿入変異を有する。
コホート1,5及び6:局所進行又は転移性NSCLCに対して過去に全身療法を受けていてはならない。
コホート2:局所進行又は転移性NSCLCに対して過去に全身療法を受けていてはならない。
コホート3及び3b:直近の治療ラインとして行ったオシメルチニブの単独投与中又は投与後に,進行が認められていなければならない。オシメルチニブは,局所進行又は転移性NSCLC に対する一次治療として,あるいは第一又は第二世代EGFR TKIによる前治療後の二次治療において単独療法として投与されていなければならない。
コホート4:被験者は,標準治療の一環として,拡大アクセスプログラムとして,又は長期継続投与試験からのロールオーバーとして,アミバンタマブIV Q2Wレジメン(体重に応じて1,050 mg又は1,400 mg)をアミバンタマブの用量減量なしに8週間以上受けている必要がある。
コホート7:直近の治療ラインとして行ったアミバンタマブとラゼルチニブの併用投与中又は投与後に,進行が認められていなければならない。アミバンタマブとラゼルチニブの併用投与は,局所進行又は転移性NSCLC に対する一次治療として実施されていなければならない。
コホート2,3,3b,及び7のみ:扁平上皮NSCLCを除外する。CLIA認定検査機関(米国)又は国・地域の認定検査機関(米国以外)で,ctDNA又は腫瘍組織を用い,FDAが承認した検査法又はバリデートされたほかの検査法によって,EGFR変異が確認されている必要がある。EGFR遺伝子変異を実証する最初の検査報告書の写しを被験者記録に含めなければならず,匿名化した写しも治験依頼者に提出する必要がある。
・コホート4を除く全てのコホート:被験者は,RECIST v1.1に従い1つ以上の測定可能病変を有すること。標的病変のみに放射線照射を施行していた場合は,放射線照射の完了以降に疾患進行の徴候が認められなければならない。未照射の測定可能病変が1つのみ存在し,生検を実施して標的病変として許容される場合,生検の14日以上後にベースラインの腫瘍評価スキャンを実施する必要がある。
・自然に又は治療により治験の安全性評価項目又は治験薬の有効性に影響を及ぼす可能性が低い,二次悪性腫瘍(治験の対象疾患を除く)の病歴又は併存症がある患者でもよい
・十分な臓器(腎臓、肝臓、血液、凝固及び心臓)機能を有する者
・米国東海岸がん臨床試験グループ(ECOG)performance statusが0又は1である
・コホート6:治験薬投与期間の最初の4カ月間は直接作用型経口抗凝固薬又は低分子量ヘパリンによる予防的抗凝固療法の使用に適格であり,これを遵守することに同意しなければならない
・被験者は,治験中及び治験薬の最終投与後の6カ月間,卵子や卵母細胞を生殖補助の目的で提供しないこと,又は将来の使用のために冷凍しないことに同意しなければならない。被験者は,抗癌剤治療により受胎能が損なわれる可能性があるため,治験薬投与前に卵子の保存を考慮すること。
- Participant must have histologically or cytologically confirmed, locally advanced or metastatic, non-small cell lung cancer (NSCLC) that is not amenable to curative therapy including surgical resection or chemoradiation. Additional Cohort specific disease requirements include: Cohorts 1, 3, 3b, 5, 6 and 7: epidermal growth factor receptor (EGFR) exon 19 deletion (Exon19del) or Exon 21 L858R mutation;
Cohort 2: EGFR Exon 20ins mutation. Cohorts 1,5,and6: Participant should not have received any prior systemic therapy for locally advanced or metastatic NSCLC. Cohort 2: Participant should not have received any prior systemic therapy for locally advanced or metastatic NSCLC.
Cohorts 3and3b: Participant must have progressed on or after osimertinib monotherapy as the most recent line of treatment. Osimertinib must have been administered as either the first-line treatment for locally advanced or metastatic disease or in the second-line setting after prior treatment with first- or second-generation EGFR tyrosine kinase inhibitor (TKI) as a monotherapy.
Cohort 4: Participants need to currently be on an amivantamab IV Q2W regimen (1,050 mg or 1,400 mg depending on weight) for at least 8 weeks, as part of standard of care, an expanded access program, or as a rollover from a longterm extension, without any amivantamab dose reduction. Cohort 7: Participants must have progressed on or after the combination of amivantamab and lazertinib as the most recent line of treatment. The combination of amivantamab and lazertinib must have been administered as the first-line treatment for locally advanced or metastatic disease.
Cohort 2, 3, 3b, and 7 only: Squamous NSCLC are excluded. EGFR mutation must have been identified as determined by Food and Drug Administration (FDA) approved or other validated
test of either circulating tumor deoxyribonucleic acid (ctDNA) or tumor tissue in a clinical laboratory improvement amendments (CLIA) certified laboratory (sites in the United states [US]) or an accredited local laboratory (sites outside of the US). A copy of the initial test report documenting the EGFR mutation must be included in the participant records and a deidentified copy must also be submitted to the sponsor
- All cohorts except Cohort 4: Participants must have at least 1 measurable lesion, according to Response Evaluation Criteria in Solid Tumors (RECIST) version 1.1. If the only target lesion has been previously irradiated, it must show signs of disease progression since radiation
was completed If only 1 non-irradiated measurable lesion exists, which undergoes a biopsy and is acceptable as a target lesion, the baseline tumor assessment scans should be performed at least 14 days after the biopsy
- May have a prior or concurrent second malignancy (other than the disease under study) which natural history or treatment is unlikely to interfere with any study endpoints of safety or the efficacy of the study treatment(s)
- Have adequate organ (renal, hepatic, hematological, coagulation and cardiac) functions
- Participant must have eastern cooperative oncology group (ECOG) status of 0 or 1
- Cohort 6: Must be eligible for, and agree to comply with, the use of prophylactic anticoagulation with a direct oral anticoagulant or a low molecular weight heparin during the first 4 months of study treatment
- A participant must agree not to donate eggs (ova, oocytes) or freeze for future use for the purposes of assisted reproduction during the study and for a period of 6 months after receiving the last dose of study treatment. Female participants should consider preservation of eggs prior to study treatment as anti-cancer treatments may impair fertility
・薬剤誘発性ILDを含む間質性肺疾患(ILD)又は放射線肺臓炎の病歴を有している患者
・被験者が組み入れられるコホートで使用する薬剤の添加物のいずれかに過敏症の病歴がある
・Cycle 1のDay 1前3カ月以内に生又は弱毒生ワクチンの接種を受けた患者。季節性インフルエンザワクチン及びCOVID-19に対する非生ワクチンは除外対象ではない。
・ラゼルチニブを含むレジメンを有する全コホート:現在,強力なCYP3A4/5誘導薬であることが知られている薬剤又はハーブ系サプリメントを投与されており,Cycle 1Day 1前に使用を中止して適切な休薬期間を確保することができない場合。
・その他の臨床的に活動性の感染性肝疾患
・以下の臨床的に重大な心血管疾患の病歴がある患者。ただしこれらに限定されない。
a. 全コホート:治験薬の初回投与前1カ月以内の深部静脈血栓症又は肺塞栓症の診断,若しくは治験薬の初回投与前6カ月以内の以下のいずれか:心筋梗塞,不安定狭心症,脳卒中,一過性脳虚血発作,冠動脈/末梢動脈バイパス移植術,又は全ての急性冠症候群。非閉塞性カテーテル関連血餅などの臨床的に重大でない血栓症は除外対象ではない。
b.ラゼルチニブを含むレジメンを有する全コホート:VTEイベントの重要な遺伝的素因(第V 因子Leiden 変異など)を有する被験者
c. ラゼルチニブを含むレジメンを有する全コホート:VTE の既往があり,NCCN 又は国・地域のガイドラインに基づき,適切な抗凝固療法を受けていない被験者
d.Fridericia 法で補正したQT 間隔(QTcF 間隔)の延長が480 msec を超える場合,又は臨床的に重大な不整脈若しくは電気生理学的疾患を有する場合(例:植込み型除細動器の留置,又は心拍数がコントロールできない心房細動)
e. コントロール不良な(持続性)高血圧:収縮期血圧>160 mmHg;拡張期血圧>100 mmHg
f. C1D1 の投与開始前6 カ月以内の,NYHA 分類III~IV 度と定義されるうっ血性心不全(CHF),又はCHF による入院(NYHA 分類を問わない)
g. 心膜炎/臨床的に重大な心嚢液貯留
h. 心筋炎
i. スクリーニング時に心エコー又はMUGA (Multigated Acquisition)スキャンで評価した,ベースラインの左室駆出率(LVEF)が治験実施医療機関の基準値下限未満
・症候性脳転移を有している場合。無症候性である,又は過去に治療されて現在は安定している脳転移を有する場合は,本治験に参加することができる。症候性又は不安定な脳転移に根治的放射線療法又は外科治療を受け,かつスクリーニング前2週間以上は臨床的に安定して無症候性である場合は,投与群への割付け前2週間以上コルチコステロイド薬の投与を受けていない,又は低用量コルチコステロイド薬(10 mg/日以下のprednisone又は同等薬)の投与を受けている場合,適格である。
- Participant has a medical history of interstitial lung disease (ILD), including drug induced ILD or radiation pneumonitis
- Participant has a history of hypersensitivity to any excipients of the investigational products to be used in their enrollment cohort
- Participant has received a live or live attenuated vaccine within 3 months before Cycle 1 Day 1. The seasonal influenza vaccine and non-live vaccines against Coronavirus disease 19 (COVID-19) are not exclusionary
- For all cohorts (with regimens potentially including lazertinib): Participant is currently receiving medications or herbal supplements known to be potent Cytochrome (CYP3A4/5) inducers and is unable to stop use for an appropriate washout period prior to Cycle 1 Day 1
- Other clinically active liver disease of infectious origin
- Participant has a history of clinically significant cardiovascular disease including, but not limited to:
a) All cohorts: diagnosis of deep vein thrombosis or pulmonary embolism within 1 month prior to the first dose of study treatment(s), or any of the following within 6 months prior to the first dose of study treatment(s): myocardial infarction, unstable angina, stroke, transient ischemic attack, coronary/peripheral artery bypass graft, or any acute coronary syndrome. Clinically non-significant
thrombosis, such as non-obstructive catheter-associated clots, are not exclusionary; b) All cohorts (with regimens potentially including lazertinib): Participant has a significant genetic predisposition to venous thromboembolic events (VTE; such as Factor V Leiden);
c) All cohorts (with regimens potentially including lazertinib): Participant has a prior history of VTE and is not on appropriate therapeutic anticoagulation as per NCCN or local guidelines; d) prolonged corrected QT interval by Fridericia (QTcF) interval greater than (>) 480 milliseconds (msec) or clinically significant cardiac arrhythmia or electrophysiologic disease (example,
placement of implantable cardioverter defibrillator or atrial fibrillation with uncontrolled rate); e) uncontrolled (persistent) hypertension: systolic blood pressure >160 millimeter(s) of mercury (mmHg); diastolic blood pressure >100 mmHg; f) Congestive heart failure defined as NYHA
class III-IV or hospitalization for congestive heart failure (CHF) (any New York Heart Association [NYHA] class) within 6 months of treatment initiation at Cycle 1/day 1 (C1D1); g) pericarditis/clinically significant pericardial effusion; h) myocarditis; i) baseline left ventricular ejection fraction (LVEF) below the institution's lower limit of normal at screening, as assessed by echocardiogram or multigated acquisition (MUGA) scan
- Participant has symptomatic brain metastases. A participant with asymptomatic or previously treated and stable brain metastases may participate in this study.
Participants who have received definitive radiation or surgical treatment for symptomatic or unstable brain metastases and have been clinically stable and asymptomatic for at least 2 weeks before Screening are eligible, provided they have been either off corticosteroid treatment or are receiving low-dose corticosteroid treatment (less than or equal to [<=] 10 milligrams per day [mg/day] prednisone or equivalent) for at least 2 weeks prior to treatment allocation

被験者にはアミバンタマブSC-CFを1サイクル28日として,Cycle 1のDay 1,8,15,及び22に,その後のCycle 2以降ではDay 1及び15に1,600 mg(体重80 kg以上の場合は2,240 mg)投与する。ラゼルチニブはCycle 1 Day 1から240mg 1日1回経口投与する。

コホート2:(Exon20 NSCLC、1L、未治療):アミバンタマブ(Q3W)+化学療法
被験者にはアミバンタマブSC-CFを1サイクル21日として,Cycle 1のDay 1に1,600 mg(体重が80 kg 以上の場合は2,240 mg),Cycle 1のDay 8及び15,その後のCycle 2以降ではDay 1に2,400 mg(体重が80 kg以上の場合は3,360 mg)を投与する。各21日サイクルのDay 1にペメトレキセド500 mg/m^2をIV投与する(ビタミンを補充)。カルボプラチンは目標濃度-時間曲線下面積(AUC)5 mg/mLの用量(AUC 5,最大750 mg)を21日サイクルのDay 1に,最大4サイクルIV投与する。

コホート3:(Exon19del/Exon21 L858R NSCLC、2L、オシメルチニブ後):アミバンタマブ(Q3W)+ラゼルチニブ+化学療法

コホート3b(Exon19del/Exon21 L858R NSCLC、2L、オシメルチニブ後):アミバンタマブ(Q3W)+化学療法
被験者にはアミバンタマブSC-CFを1サイクル21日として,Cycle 1のDay 1に1,600 mg(体重が80 kg 以上の場合は2,240 mg),Cycle 1のDay 8及び15,その後のCycle 2 以降ではDay 1に2,400 mg(体重が80 kg以上の場合は3,360 mg)を投与する。カルボプラチンは目標濃度-時間曲線下面積(AUC)5 mg/mLの用量(AUC 5,最大750 mg)を21日サイクルのDay 1に,最大4サイクルIV投与する。各21日サイクルのDay 1にペメトレキセド500 mg/m^2をIV投与し(ビタミンを補充)、疾患進行まで継続する。

コホート4:(アミバンタマブIVによる前治療歴あり):アミバンタマブIVからSC-CFへの切り替え(Q2W)
被験者には、アミバンタマブSC-CF 1,600 mg(体重80 kg以上の場合は2,240 mg)を手
動注入により投与する。

コホート5:(Exon19del/Exon21 L858R NSCLC、1L、未治療):アミバンタマブ(Q4W)+ラゼルチニブ
被験者には、Cycle 1のDay 1,8,15,及び22にアミバンタマブSC-CF 1,600 mg(体重80 kg以上の場合は2,240 mg)を導入投与する。Cycle 2のDay 1からは4週(28日)ごとにアミバンタマブSC-CF(アミバンタマブ160 mg/mLにrHuPH20を配合)3,520 mg(体重80 kg以上の場合は4,640 mg)を手動注入により投与する。ラゼルチニブはCycle 1 Day 1から240mg1日1回経口投与する。

コホート6:(Exon19del/Exon21 L858R NSCLC、1L、未治療):アミバンタマブ(Q2W)+ラゼルチニブ+抗凝固
被験者にはアミバンタマブSC-CFを1サイクル28日として,Cycle 1のDay 1,8,15,及び22に,その後のCycle 2以降ではDay 1及び15に1600 mg(体重80 kg以上の場合は2,240 mg)投与する。ラゼルチニブはCycle 1 Day 1から240mg 1日1回経口投与する。被験者は、アミバンタマブ及びラゼルチニブ投与期間の最初の4カ月間(Day 1~120)に直接作用型経口抗凝固薬又は低分子量ヘパリンによる予防的抗凝固療法も実施する。

コホート7:(Exon19del/Exon21 L858R NSCLC、2L、アミバンタマブ+ラゼルチニブ後):アミバンタマブ(Q3W)+化学療法
被験者にはアミバンタマブSC-CFを1サイクル21日として,Cycle 1のDay 1に1,600 mg(体重が80 kg以上の場合は2,240 mg),Cycle 1のDay 8及び15,その後のCycle 2以降ではDay 1に2,400 mg(体重が80 kg 以上の場合は3,360 mg)を投与する。カルボプラチンは目標濃度-時間曲線下面積(AUC)5 mg/mLの用量(AUC 5,最大750 mg)を21日サイクルのDay 1に,最大4サイクルIV投与する。各21日サイクルのDay 1にペメトレキセド500 mg/m^2をIV投与し(ビタミンを補充)、疾患進行まで継続する。

アミバンタマブは手動注入により皮下投与する。
ラゼルチニブは経口錠剤として投与する。
カルボプラチンは点滴静注する。
ペメトレキセドは点滴静注する。
直接作用型経口抗凝固薬(DOAC)は経口投与する。
低分子量ヘパリン(LMWH)は皮下投与する。
Cohort 1(Exon19del/Exon21 L858R NSCLC, 1L, Previously Untreated): Amivantamab (Q2W) + Lazertinib
Participants will receive amivantamab SC-CF injection, 1600 milligrams (mg) or 2240 mg if body weight is greater than or equal to (>=) 80 kilograms (kg), on Cycle 1 Days 1, 8, 15, and 22 and on Days 1 and 15 of each subsequent 28-day cycle, starting with Cycle 2, along with lazertinib 240 mg orally once daily.

Cohort 2(Exon20 NSCLC,1L, Previously Untreated): Amivantamab (Q3W) + Chemotherapy
Participants will receive Amivantamab SC-CF injection 1600 mg or 2240 mg if body weight is >=80 kg on Cycle 1 Day 1, 2400 mg or 3360 mg if body weight is >=80 kg on Cycle 1 Day 8 and 15 and on Day 1 of each subsequent 21-day cycle, starting with Cycle 2 along with pemetrexed 500 milligrams per meter square (mg/m^2) as intravenous (IV) infusion (with vitamin supplementation) on Day 1 of each 21-day cycle and IV infusion carboplatin area under the concentration-time curve 5 milligrams per milliliters (mg/mL) per minute (AUC 5) maximum 750 mg on Day 1 of each 21-day cycle, for up to 4 cycles.

Cohort 3(Exon19del/Exon21 L858R NSCLC,2L,Post Osimertinib):Amivantamab(Q3W)+Lazertinib+Chemotherapy
Participants will receive amivantamab SC-CF injection 1600 mg or 2240 mg if body weight is >=80 kg on Cycle 1 Day 1, 2400 mg or 3360 mg if body weight is >=80 kg on Cycle 1 Day 8 and 15 and on Day 1 of each subsequent 21-day cycle starting with Cycle 2; in combination with IV infusion carboplatin area under the concentration-time curve 5 mg/mL per minute (AUC 5) maximum 750 mg on Day 1 of each 21-day cycle, for up to 4 cycles; and pemetrexed 500 mg/m^2 as an IV infusion (with vitamin supplementation) on Day 1 of each 21-day until disease progression. Lazertinib 240 mg orally once daily starting Cycle 5 Day 1 when carboplatin is complete or sooner if carboplatin discontinued earlier than Cycle 4.

Cohort 3b(Exon19del/Exon21 L858R NSCLC, 2L, Post Osimertinib): Amivantamab (Q3W)+Chemotherapy
Participants will receive amivantamab SC-CF injection 1600 mg or 2240 mg if body weight is >=80 kg on Cycle 1 Day 1, 2400 mg or 3360 mg if body weight is >=80 kg on Cycle 1 Day 8 and 15 and on Day 1 of each subsequent 21-day cycle starting with Cycle 2; in combination with IV infusion carboplatin area under the concentration-time curve 5 mg/mL per minute (AUC 5) maximum 750 mg on Day 1 of each 21-day cycle, for up to 4 cycles; and pemetrexed 500 mg/m^2 as an IV infusion (with vitamin supplementation) on Day 1 of each 21-day until disease progression.

Cohort 4(Previously Treated with Amivantamab IV): Switch from Amivantamab IV to SC-CF (Q2W)
Participants will receive amivantamab SC-CF injection 1600 mg and 2240 mg if body weight is greater than or equal to 80 kg.

Cohort 5(Exon19del/Exon21 L858R NSCLC, 1L, Previously Untreated): Amivantamab (Q4W) + Lazertinib
Participants will receive amivantamab SC-CF induction with 1,600 mg (or 2,240 mg if BW >=80 kg) on Cycle 1 Days 1, 8, 15, and 22, starting with Cycle 2 on Day 1 of each next 28-day cycle, amivantamab SC-CF (160 mg/mL co-formulated with rHuPH20) by manual injection at 3,520 mg (or 4,640 mg if BW >=80 kg); along with lazertinib 240 mg by mouth once daily from Cycle 1 Day 1.

Cohort6(Exon19del/Exon21L858R,NSCLC1L,PreviouslyUntreated):Amivantamab(Q2W)+Lazertinib+Anticoagulant
Participants will receive will receive amivantamab SC-CF injection, 1600 milligrams (mg) and 2240 mg if body weight is greater than or equal to (>=) 80 kilograms (kg), on Cycle 1 Days 1, 8, 15, and 22 and on Days 1 and 15 of each subsequent 28-day cycle, starting with Cycle 2, along with lazertinib 240 mg orally once daily from Cycle 1 Day 1. Participants will additionally take prophylactic anticoagulation with a direct oral anticoagulant (DOAC) or a low molecular weight heparin (LMWH) for the first four months of study treatment (from Day 1 through Day 120) with the combination of amivantamab and lazertinib.

Cohort 7(Exon19del/Exon21 L858R NSCLC,2L,Post Amivantamab+Lazertinib):Amivantamab(Q3W)+Chemotherapy

Each of the domain/scale scores is scored on a 1-100 scale, where 0 is worst and 100 is best.

Patient-reported status as assessed by PGIC scale score will be reported. The PGIC is an assessment of the participant's overall sense of whether there has been a change since starting treatment. The PGIC is a 7-point response scale. Participants will be asked to rate their current fatigue as compared to when they started the study, using the following 7-point scale: 1 = Much better, 2 = Moderately better, 3 = A little better, 4 = No change, 5 = A little worse, 6 = Moderately worse, and 7 = Much worse.

Cohort 4: Patient-reported Status as Assessed by Patient Global Impression of Symptom Severity (PGIS) Scale Score

Patient-reported status as assessed by PGIS scale score will be reported. The PGIS is an assessment of lung cancer severity at a given point in time. The PGIS is a 5-point response scale. Participants will be asked to rate their fatigue over the past 7 days using the following 5-point scale: 1 = None, 2 = Mild, 3 = Moderate, 4 = Severe, and 5 = Very severe.
