本試験は再発又は難治性CD20陽性非ホジキンリンパ腫の成人患者を対象に、治験薬TAK-981とリツキシマブの併用療法を検討する試験である。

1. 患者集団:
a. 第1相Dose escalation 及び日本Specific Lead-in:
• マントル細胞リンパ腫、低悪性度リンパ腫(濾胞性又はそれ以外)から形質転換したびまん性大細胞型B細胞リンパ腫(DLBCL)、骨髄の小細胞浸潤を伴うDLBCL、DLBCLとバーキットリンパ腫の中間の特徴を有するか若しくはDLBCLとホジキンリンパ腫の中間の特徴を有するB細胞リンパ腫、グレード3Bの濾胞性リンパ腫(FL)、分類不能の高悪性度B細胞リンパ腫を含むアグレッシブ非ホジキンリンパ腫(aNHL)患者。これらの患者は、リツキシマブ、シクロホスファミド、ドキソルビシン(ヒドロキシダウノルビシン)、ビンクリスチン(オンコビン)及びprednisone による前治療歴(又は同等の抗CD20モノクローナル抗体を含む治療)を有し、再発又は難治性(r/r)となってさらに1ラインの前治療歴を有する必要がある。
• リツキシマブ又は他の抗CD20モノクローナル抗体に抵抗性のインドレント非ホジキンリンパ腫(iNHL)(グレード1~3AのFL及び辺縁帯リンパ腫を含む)で、r/r iNHLに対する全身療法を1回以上受けたことがある患者。
• リツキシマブ又は抗CD20に対する抵抗性は、リツキシマブ/抗CD20を含むレジメン(単剤療法又は化学療法との併用)による前治療が無効若しくは治療中に進行した場合、又はリツキシマブ若しくは抗CD20の最終投与後6ヵ月以内に進行した場合と定義する。
注意:リツキシマブ/抗CD20の最小有効投与は1サイクルの完了(すなわち、週1回(QW)×4回の単剤療法又は化学療法と併用した場合は1回の完全投与)とする。抗CD20抗体又は細胞傷害性薬剤による前治療は、単剤又は併用療法の構成要素として投与されていてもよい。同じ単剤又は併用療法のコースを反復した場合は、独立したレジメンとみなす。
b. 第2相:以下のCD20 陽性が確認された非ホジキンリンパ腫
• DLBCLの治療薬として規制当局から承認を受けたキメラ抗原受容体(CAR)-T細胞療法後に進行又は再発したr/r DLBCL患者(コホートA)。
• 2ライン以上3ライン以下の全身療法後に進行又は再発し、細胞療法を受けたことがないr/r DLBCL患者。前治療には、少なくとも1ラインのCD20標的療法が含まれていなければならない(コホートB)。
• 2ライン以上3ライン以下の全身療法後に進行又は再発したr/r FL患者。前治療には、少なくとも1ラインのCD20標的療法が含まれていなければならない(コホートC)。

2. 治験責任医師又は治験分担医師が自家造血幹細胞移植(ASCT)に不適格と判断した患者、又はASCTを拒否した患者。
3. Eastern Cooperative Oncology Group(ECOG)のperformance scoreが2以下の患者。
4. 実施医療機関の臨床検査基準値に基づき、スクリーニング時に以下の十分な骨髄機能を有する患者。
• 血小板数が75.0×109/L以上、グレード2の血小板減少症(血小板数が50.0×109/L以上)は、明らかに骨髄浸潤に起因し、骨髄異形成症候群又は骨髄形成不全のエビデンスがない場合には許容される。好中球絶対数(ANC)が1.0×109/L以上。ヘモグロビンが85 g/L以上(評価の14日以上前の赤血球(RBC)輸血は許容される)。
5. 実施医療機関の臨床検査基準値に基づき、スクリーニング時に以下の十分な腎機能及び肝機能を有する患者。
• Cockcroft-Gault式により算出したクレアチニンクリアランスが30 mL/min以上。
・カリウム値が基準値下限(LLN)以上。カリウムが基準値上限(ULN)を超える場合は、治験依頼者のメディカルモニター/指名された者と協議することが望ましい。
• アスパラギン酸アミノトランスフェラーゼ及びアラニンアミノトランスフェラーゼが治験実施施設の基準値上限(ULN)の3.0倍以下、ビリルビンがULNの1.5倍以下。ジルベール症候群の患者は、治験責任医師又は治験分担医師とメディカルモニターの協議に基づき、ビリルビン値がULNの1.5倍を超えても許容される。
6. 心エコー又はマルチゲートスキャン(MUGA)で計測した左室駆出率(LVEF)が40%以上の患者。
7. 安全な薬剤投与及び本治験で必要な薬物動態(PK)及び薬力学解析用の検体採取のために適切な静脈ルートが確保可能な患者。
8. コンピューター断層撮影(CT)によるLugano分類に基づき二次元的に測定可能な病変を1つ以上有する患者。以前の放射線治療部位に存在する腫瘍病変は、その病変に進行が認められた場合に測定可能とする。
9. 第1相のDose escalation で必須の投与前皮膚生検1回及び投与期間中の皮膚生検1回に同意する意思がある患者。標的結合の薬力学的エビデンスが十分に得られた時点で、治験依頼者が皮膚生検に関する登録条件を中止することがある。
10. 第2相に登録された患者については、可能であれば、スクリーニング前12 ヵ月以内に採取した保存用腫瘍組織の提出が必須である。
11. 過去の治療のすべての毒性作用からグレード1、ベースライン又は後遺症として定着するまで回復している患者〔脱毛症、ニューロパチー、安定した内分泌補充療法を受けている自己免疫性内分泌障害、神経毒性(グレード1又は2は許容される)、又は骨髄パラメータ(グレード1又は2のうち、骨髄浸潤と直接関連するものは許容される)を除く〕。
1. Participant Population:
a. For Phase 1 Dose Escalation and Japan-specific Lead-in:
- Aggressive Non-Hodgkin Lymphoma (aNHL) including mantle cell lymphoma and diffuse large B-cell lymphoma (DLBCL) histologies such as transformed DLBCL from low-grade lymphoma (follicular or others), DLBCL associated with small-cell infiltration in bone marrow, B-cell lymphoma with intermediate features between DLBCL and Burkitt's lymphoma or with intermediate features between DLBCL and Hodgkin lymphoma, follicular lymphoma (FL) grade 3B, and aggressive B-cell lymphoma unclassifiable who must have previously received rituximab, cyclophosphamide, doxorubicin (hydroxydaunorubicin), vincristine, (Oncovin) and prednisone (R-CHOP) (or equivalent anti-CD20 containing therapy) and 1 additional line of therapy in the relapsed or refractory (r/r) setting.
- Indolent non-Hodgkin lymphoma (iNHL) (including FL of grades 1-3A and marginal zone lymphoma) refractory to rituximab or to any other anti-CD20 monoclonal antibody, who have received at least 1 prior systemic therapy for r/r iNHL:
- Rituximab or anti-CD20 refractoriness is defined as failure to respond to, or progression during, any previous rituximab/anti-CD20-containing regimen (monotherapy or combined with chemotherapy), or progression within 6 months of the last rituximab or anti-CD20 dose.
Note: The minimum qualifying rituximab/anti-CD20 dose is 1 full cycle (that is, weekly*4 doses monotherapy or 1 complete dose if combined with chemotherapy). Prior anti-CD20 antibody or cytotoxic drugs may have been administered as single agents or as components of combination therapies. Each repeated course of the same single-agent or combination is considered an independent regimen.
b. For Phase 2, the following confirmed CD20 +:
- r/r DLBCL progressed or relapsed after a prior chimeric antigen receptor (CAR) T-cells therapy that has received approval by a health authority for the treatment of DLBCL (Cohort A).
- r/r DLBCL that has progressed or relapsed after at least 2 but no more than 3 prior lines of systemic therapy and has not I prior cellular therapy. At least one prior line of therapy must have included a CD20-targeted therapy (Cohort B).
- r/r FL that has progressed or relapsed after at least 2 but no more than 3 prior lines of systemic therapy. At least 1 prior line of therapy must have included a CD20-targeted therapy (Cohort C).

2. Must be considered ineligible in the opinion of the investigator, or refused autologous stem-cell transplantation (ASCT).
3. Eastern Cooperative Oncology Group (ECOG) performance score of less than or equal to (<=) 2.
4. Adequate bone marrow function per local laboratory reference range at screening as follows:
- Platelet count greater than or equal to (>=) 75.0*10^9/L, Grade 2 thrombocytopenia (platelet count >=50.0*10^9 per liter [/L]) is allowed if it is clearly due to marrow involvement with no evidence of myelodysplastic syndrome or hypoplastic bone marrow if found. Absolute neutrophil count (ANC) >=1.0*10^9/L. Hemoglobin >=85 gram per liter (g/L) (red blood cell [RBC] transfusion allowed >=14 days before assessment).
5. Adequate renal and hepatic function, per local laboratory reference range at screening as follows:
- Calculated creatinine clearance >=30 milliliter per minute (mL/min) calculated with Cockcroft-Gault formula.
- Potassium levels >=lower limit of normal (LLN). For potassium > upper limit of normal (ULN) discussion with Takeda medical monitor (MM)/designee recommended.
- Aspartate aminotransferase and alanine aminotransferase <=3.0*the ULN of the institution's normal range; bilirubin <=1.5*ULN. Participants with Gilbert's syndrome may have a bilirubin level >1.5*ULN, per discussion between the investigator and the medical monitor.
6. Left ventricular ejection fraction (LVEF) >=40 percent (%); as measured by echocardiogram or multiple gated acquisition (MUGA) scan.
7. Suitable venous access for safe drug administration and the study-required pharmacokinetic (PK) and pharmacodynamic sampling.
8. Have at least 1 bidimensionally measurable lesion per Lugano Classification by computed tomography (CT). Tumor lesions situated in a previously irradiated area are considered measurable if progression has been demonstrated in such lesions.
9. Willing to consent to 1 mandatory pretreatment and 1 on-treatment skin biopsy during Phase 1. The skin biopsy entry requirement may be discontinued by the sponsor once there is enough pharmacodynamic evidence of target engagement.
10. For participants enrolled in Phase 2, if available, mandatory submission of archival tumor tissue acquired <=12 months prior to screening.
11. Recovered to Grade 1, baseline or established as sequela, from all toxic effects of previous therapy (except alopecia, neuropathy, autoimmune endocrinopathies with stable endocrine replacement therapy, neurotoxicity [Grade 1 or 2 permitted], or bone marrow parameters [any of Grade 1, 2, permitted if directly related to bone marrow involvement]).
1. 中枢神経系(CNS)リンパ腫で、腰椎穿刺による細胞診が陽性又は CTスキャン/磁気共鳴画像法(MRI)により、活動性の脳転移又は軟膜転移が認められる患者。
2. リツキシマブによる前治療で投与中止に至ったグレード3以上の注入に伴う反応(IRR)の既往歴を有する患者。
3. 移植後リンパ増殖性疾患を有する患者。ただし、ASCT後の再発性非ホジキンリンパ腫(NHL)は除く。
4. TAK-981投与前12週間以内にASCT又はCAR Tなどの細胞療法を受けた患者。
5. 同種造血幹細胞移植を受けたことがある患者。
6. 白血病を発症しているリンパ腫患者。
7. 化学療法、ホルモン療法、又は治験薬を含む抗がん療法を、TAK-981投与前2週間又は半減期の5倍以上に相当する期間のいずれか短い期間内に受けた患者。低用量ステロイド(経口prednisone又は同等の薬剤、20 mg/日以下)、前立腺癌又は乳癌に対するホルモン療法(アジュバント療法として)、並びにビスホスホネート及び核内因子活性化受容体リガンドkappa-B(RANKL)阻害薬の投与は許容される。
8. 治験薬の初回投与前 14日以内に大手術を受け、手術による合併症から十分に回復していない患者。
9. 治験責任医師又は治験分担医師及び治験依頼者により、本治験への参加のリスク/ベネフィット比を大幅に上昇させると判断された重大な医学的疾患又は状態の患者。これには、過去6ヵ月以内の急性心筋梗塞又は不安定狭心症、コントロール不良の糖尿病、重大な活動性細菌、ウイルス又は真菌感染症、重度の免疫不全状態、重度の非代償性高血圧及びニューヨーク心臓協会心機能分類クラスIII又はIVのうっ血性心不全、グレード2を超える進行中の症候性不整脈、肺塞栓症、症候性脳血管イベント、その他の重篤な心臓の疾患(例えば、心嚢液貯留や拘束型心筋症)が含まれるが、これらに限定されない。安定した抗凝固療法を受けている慢性心房細動は許容される。
10. 慢性C型肝炎が判明している患者及び/又は慢性B型肝炎に対する血清検査陽性(ワクチン接種又は免疫グロブリン(Ig)療法による受動免疫に起因する場合を除く)が判明している患者。HIV感染が判明している患者。
11. 過去3年以内に二次性悪性腫瘍が認められた患者。ただし、治療済みの基底細胞癌又は限局性の扁平上皮皮膚癌、限局性前立腺癌、子宮頸部上皮内癌、切除済みの結腸直腸腺腫性ポリープ、乳房の上皮内癌、又は積極的な抗がん治療を受けていないその他の悪性腫瘍を除く。
12. 治験薬投与開始前4週間以内に生ワクチンの接種を受けた患者。
13. 20 mgを超えるprednisone又は同等の薬剤、細胞傷害性薬剤又は生物学的製剤を必要とするコントロール不良の活動性自己免疫疾患を有する患者。
14. 治験薬の初回投与前1週間以内にコルチコステロイドを使用した患者。ただし、喘息に対する吸入ステロイド薬、局所ステロイド薬の使用、治験薬又は造影剤投与のための前投薬など、他の医学的状況のために必要な場合を除く。Prednisone換算で1 日量が20 mgを超えるステロイドの全身曝露が必要な患者、又はリンパ腫のコントロール又は白血球数の減少のためにステロイドの投与を受けている患者は不適格とする。
15. ベースライン時にFridericia補正法によるQT 間隔(QTcF)の延長がみられる患者(例:補正QT 間隔[QTc]が470msを超える女性患者及び450msを超える男性患者、先天性QT 延長阻害薬症候群の既往又はトルサード・ド・ポアント)。
16. CYP 3A4/5の強力な若しくは中等度の阻害薬及び誘導薬であることが知られている薬剤、並びに強力なP-糖蛋白質(Pgp)阻害薬を投与中の患者、又は継続使用が必要である患者。このような患者が本治験に参加する場合には、TAK-981の投与前に少なくとも2週間(CYP3A4/5阻害薬及びPgp 阻害薬は1週間)はこれらの薬剤の使用を中止する必要がある。
17. ドイツの法律に従い、司法当局又は行政当局から出された命令により施設に収容されているドイツの患者。
1. Central nervous system lymphoma; active brain or leptomeningeal metastases, as indicated by positive cytology from lumbar puncture or CT scan/magnetic resonance imaging (MRI).
2. History of Grade >=3 infusion-related reaction (IRR) that lead to permanent discontinuation of previous rituximab treatment.
3. Post transplantation lymphoproliferative disease except relapsed NHL after ASCT.
4. Undergone ASCT or treatment with cellular therapy including CAR T within <=12 weeks of TAK-981 dosing.
5. Prior allogeneic hematopoietic stem-cell transplantation.
6. Lymphomas with leukemic expression.
7. Prior anticancer therapy including chemotherapy, hormonal therapy, or investigational agents within 2 weeks or within at least 5 half-lives before TAK-981 dosing, whichever is shorter. Low dose steroids (oral prednisone or equivalent <=20 mg per day), hormonal therapy for prostate cancer or breast cancer (in adjuvant situation), and treatment with bisphosphonates and receptor activator of nuclear factor kappa-B ligand (RANKL) inhibitors are allowed.
8. Major surgery within 14 days before the first dose of study drug and not recovered fully from any complications from surgery.
9. Significant medical diseases or conditions, as assessed by the Investigators and Sponsor that would substantially increase the risk-benefit ratio of participating in the study. This includes but is not limited to acute myocardial infarction or unstable angina within the last 6 months, uncontrolled diabetes mellitus, significant active bacterial, viral or fungal infections, severely immunocompromised state, severe non-compensated hypertension and congestive heart failure New York Heart Association Class III or IV, ongoing symptomatic cardiac arrhythmias of >Grade 2, pulmonary embolism, or symptomatic cerebrovascular events, or any other serious cardiac condition (example, pericardial effusion or restrictive cardiomyopathy). Chronic atrial fibrillation on stable anticoagulant therapy is allowed.
10. Known chronic hepatitis C and/or positive serology (unless due to vaccination or passive immunization due to immunoglobulin [Ig] therapy) for chronic hepatitis B. Known Human Immunodeficiency Virus (HIV) infection.
11. Second malignancy within the previous 3 years, except treated basal cell or localized squamous skin carcinomas, localized prostate cancer, cervical carcinoma in situ, resected colorectal adenomatous polyps, breast cancer in situ, or other malignancy for which the participant is not on active anticancer therapy.
12. Receipt of any live vaccine within 4 weeks of initiation of study treatment.
13. Active, uncontrolled autoimmune disease requiring >20 mg of prednisone or equivalent, cytotoxics or biologicals.
14. Corticosteroid use within 1 week before the first dose of study drug, except as indicated for other medical conditions such as inhaled steroid for asthma, topical steroid use, or as premedication for administration of study drug or contrast. Participants requiring steroids at daily doses >20 mg prednisone equivalent systemic exposure daily, or those who are administered steroids for lymphoma control or white blood cell count lowering are not eligible.
15. With baseline prolongation of the QT interval with Fridericia correction method (QTcF) (example, >470 millisecond (ms) for women and >450 ms for men and a history of congenital long QT syndrome, or torsades de pointes).
16. Receiving or requiring the continued use of medications that are known to be strong or moderate inhibitors and inducers of Cytochrome P450 3A4/5 (CYP3A4/5) and strong P-glycoprotein (Pgp) inhibitors. To participate in this study, such participants should discontinue use of such agents for at least 2 weeks (1 week for CYP3A4/5 and Pgp inhibitors) before receiving a dose of TAK-981.
17. Participants in Germany who are committed to an institution by virtue of an order issued either by judicial or administrative authorities as per German law.

Phase 1(aNHL/iNHL対象):TAK-981(10-160 mg)+ リツキシマブ 375 mg/m^2
TAK-981(用量は10 mgから160 mgまで漸増又は漸減)を1サイクル21日間の1日目及び8日目に週1回、又は1、4、8 及び11 日目に週2回(BIW)静脈内投与し、リツキシマブ 375 mg/m^2をサイクル1の1、8、15日目に1回ずつ、サイクル2以降は21日間サイクルのうち1日目に1回、最長12ヵ月間、疾患進行(PD)の確定、若しくは許容できない毒性の発現まで静脈内投与する。日本Specific Lead-inの被験者には21日サイクルの1、4、8 及び11日目にTAK-981 を単剤投与する。TAK-981 開始用量で忍容性が確認された後、リツキシマブとの併用投与コホートの被験者にはDose escalationで決定した第2 相試験推奨用量(RP2D) までTAK-981 を漸増投与する。投与量は安全性、薬物動態、薬力学的データから判断し漸増する。

Phase 2、コホートA: CAR-T細胞療法後に進行した、r/rDLBCL患者対象:TAK-981+リツキシマブ
DLBCLの治療として規制当局から承認を受けたキメラ抗原受容体(CAR)T細胞療法後に進行又は再発した、r/rDLBCL患者を対象に、TAK-981(RP2D)を1サイクル21日間の1日目及び8日目に週1回、又は21 日サイクルに週2回静脈内投与し、リツキシマブをサイクル1の1、8、15日目に1回ずつ、サイクル2以降は21日間サイクルのうち1日目に1回、最長12ヵ月間、PDの確定、若しくは許容できない毒性の発現まで静脈内投与する。

Phase 2、コホートB:2~3 ラインの全身療法後に進行又は再発した、CAR T細胞療法の治療歴のないr/rDLBCL患者対象:TAK-981+リツキシマブ
CAR T細胞療法の治療歴がなく、2ライン以上3ライン以下の全身療法後に進行又は再発したr/rのDLBCL患者を対象に、TAK-981(2用量)を1サイクル21日間の1日目及び8日目に週1回、又は21 日サイクルに週2回静脈内投与し、リツキシマブをサイクル1の1、8、15日目に1回ずつ、サイクル2以降は21日間サイクルのうち1日目に1回、最長12ヵ月間、PDの確定、若しくは許容できない毒性の発現まで静脈内投与する。

Phase 2、コホートC:2~3 ラインの全身療法後に進行又は再発した、r/rFL患者対象
CAR T細胞療法の治療歴がなく、2ライン以上3ライン以下の全身療法後に進行又は再発したr/rFL患者を対象に、TAK-981(2用量)を1サイクル21日間の1日目及び8日目に週1回静脈内投与し、リツキシマブをサイクル1の1、8、15日目に1回ずつ、サイクル2以降は21日間サイクルのうち1日目に1回、PDの確定、許容できない毒性の発現まで静脈内投与する。
Phase 1, aNHL/iNHL: TAK-981 (10-160 mg) + Rituximab 375 mg/m^2
TAK-981 (at increasing dose levels from 10 milligram [mg] to 160 mg), infusion, intravenously, once weekly (QW) on Days 1 and 8 OR on Days 1, 4, 8, and 11 twice weekly (BIW) every 21 days in each 21- day treatment cycle in combination with rituximab 375 milligram per square meter (mg/m^2), infusion, intravenously, once on Days 1, 8, and 15 of Cycle 1 and then on Day 1 of each 21-day treatment cycle from Cycle 2 for up to 12 months or disease progression (PD) or unacceptable toxicity. Participants in the Japan-specific Lead-in expansion will be treated with TAK-981 single agent on Days 1, 4, 8 and 11 in treatment cycles of 21 days. Once the starting dose of TAK-981 has been deemed safe, participants in the combination with rituximab cohorts will be treated with increasing doses of TAK-981 up to the recommended Phase 2 dose (RP2D) and schedule determined in the Dose Escalation. Dose levels will be escalated based on available safety, PK and pharmacodynamic data.

Phase 2, Cohort A r/r DLBCL Progressed to CAR T-cell therapy: TAK-981+Rituximab
TAK-981 RP2D, infusion, intravenously, QW on Days 1 and 8 BIW every 21 days in each 21-day treatment cycle in combination with rituximab infusion, intravenously, once on Days 1, 8, and 15 of Cycle 1 and then on Day 1 of each 21-day treatment cycle from Cycle 2 for up to 12 months or PD or unacceptable toxicity in participants with r/r DLBCL progressed or relapsed after a prior CAR T-cells therapy that has received approval by a health authority for the treatment of DLBCL.

Phase 2, Cohort B: r/r DLBCL; no CAR T-cell Therapy;2-3 Prior Lines: TAK-981+Rituximab
TAK-981 infusion (to optimize two dose levels), intravenously, QW on Days 1 and 8 every 21 days in each 21 day treatment cycle in combination with rituximab, infusion, intravenously, once on Days 1, 8, and 15 of Cycle 1 and then on Day 1 of each 21-day treatment cycle from Cycle 2 for up to 12 months or PD or unacceptable toxicity in participants with r/r DLBCL that has progressed or relapsed after at least 2 but no more than 3 prior lines of systemic therapy and no prior therapy with CAR T-cells.

Phase 2, Cohort C: r/r FL; no CAR T-cell Therapy;2-3 Prior Lines: TAK-981+Rituximab
TAK-981 infusion (to optimize two dose levels), intravenously, QW on Days 1 and 8 every 21 days in each 21 day treatment cycle in combination with rituximab infusion, intravenously, once on Days 1, 8, and 15 of Cycle 1 and then on Day 1 of each 21-day treatment cycle from Cycle 2 for up to PD or unacceptable toxicity in participants with r/r FL that has progressed or relapsed after at least 2 but no more than 3 prior lines of systemic therapy and no prior therapy with CAR T-cells.

PFSは治験薬初回投与日からPDが最初に確認された日又は理由を問わない死亡が認められた日のいずれか早い方までの期間と定義する。PDはリンパ腫患者を対象としたリンパ腫の治療効果判定基準を用いて判定する。PFSは治験責任医師がリンパ腫のLugano分類に基づき評価する。
