２　臨床研究の目的及び内容並びにこれに用いる医薬品等の概要

（１）臨床研究の目的及び内容

痔核手術の対象となる患者に対して、患部付近の冷却による局所麻酔穿刺時疼痛の緩和効果を明らかにすることである。

1. jRCT公表日から2023年3月31日の間に痔核の手術を受ける予定である患者
2. 同意取得時の年齢が20歳以上である
3. 本人の自由意思により文書による同意を得られる患者
4. ゴリガー分類Ⅲ度またはⅣ度の痔核を有する患者
5. それらの痔核が左右それぞれに1個ずつ位置している患者		 1. The patient who is going to be operated on for hemorrhoids between an ethic jury approval day and January 31, 2023
2. Age at the time of the agreement acquisition is 20 years or older
3. The patient that a written agreement is got by the free will of the person
4. Patient having the hemorrhoids of the Gori garfish classification grade III or IV degree
5. The patient who has hemorrhoids by one in each right and left
1. 肛門手術の既往がある患者
2. 妊娠中、授乳中の女性患者
3. 認知症や精神障害、認知障害等を伴う疾患を有する等により、有効なインフォームド・
コンセントを与えられない患者
4. リドカインの成分又はアミド型局所麻酔薬に対し過敏症の既往歴のある
5. 痔瘻または裂肛を合併している患者

担当医が試験治療を中止すべきと判断した場合。

本研究はクロスオーバー試験であり、麻酔注射の順序が異なる2群に無作為に分ける。Group1は冷却側の麻酔を先行して、非冷却側の麻酔を後で行う。Group2は非冷却側の麻酔を先行して、冷却側の麻酔を後で行う。
Group1に割り付けられた患者は、冷却側の痔核周辺に保冷剤を２分間当ててから、1回目の局所麻酔薬の注射を冷却側の痔核周辺に行う。
Washout periodである2分が経過したら、２回目の局所麻酔薬の注射を非冷却側の痔核周辺に行う。
一方、Group2に割り付けられた患者は、1回目の局所麻酔薬の注射を非冷却側の痔核周辺に行う。次に、冷却側の痔核周辺に保冷剤を２分間当ててから、２回目の局所麻酔薬を冷却側の痔核周辺に行う。
局所麻酔薬として1%Lidocaine（1筒10ml）を冷却側と非冷却側のそれぞれに対して10mlずつ皮下注射する。ただし、麻酔薬の種類、投与量、部位に関しては通常診療の範囲内とし無作為化は行わない。		 This trial is a crossover design, and the order of anesthetic injections divides it into different two groups at random. In the case of Group1, we lead by anesthesia on the cooling side and perform anesthesia on the non-cooling side later. In the case of Group2, we lead by anesthesia on the non-cooling side and perform anesthesia on the cooling side later. The patient allocated for Group1 performs the injection of the first local anesthetic around hemorrhoids on the cooling side after putting an ice pack around hemorrhoids on the cooling side for two minutes. Then we give the injection of the second local anesthetic around hemorrhoids on the non-cooling side when two minutes that are washout period pass.
On the other hand, the patient allocated for Group2 performs the injection of the first local anesthetic around hemorrhoids on the non-cooling side. Then, we perform the second local anesthetic around hemorrhoids on the cooling side after having put an ice pack around hemorrhoids on the cooling side for two minutes.
We inject 1%Lidocaine (10 ml of 1 pipe) subcutaneously by 10 ml for each on the cooling side and the non-cooling side as a local anesthetic. But we do not randomize it about a kind, a dose, a side of the anesthetic.
